Outreach Worker

Job Overview:

The Outreach Worker will provide outreach in the community for patients requesting Family Planning Services. The outreach worker will complete a brief intake assessment and be able to assist patients with scheduling appointments and communication with financial consultant within the Family Planning Clinic. The outreach worker will distribute approved Family planning educational materials and clinic contact information within Monmouth and Ocean Counties. The outreach worker will communicate with the clinical and medical director on a weekly basis and be available during clinic hours as needed. Participate in health fairs and community neighborhood events. Reaches out to underserved populations in Monmouth and Ocean Counties.

Qualifications:

Required:

• High school Diploma or equivalent

Preferred:

• Excellent communication and interpersonal skills
• Independence, requiring little supervision in the community
• Able to communicate in multiple languages ie, English, Spanish and Brazilian Portuguese preferred
• Able to use I-pad/lap top computer
• Familiar with community to be served
