Our integrated programs and services offer coordinated care to people of all ages in their homes, the community, and in our clinics. We strive for excellence in clinical care as we serve those living with mental illness, substance use disorders, crisis, unstable housing, and trauma. Canvas Health is a non-profit, Certified Community Behavioral Health Clinic (CCBHC) specializing in serving clients who are on Medicaid. Position Summary This position provides planning, coordination and monitoring of mental health services to adults with serious and persistent mental illnesses who are eligible for Rule 79 or Minnesota Community Healthcare Network case management services. It is collaborative in nature and requires working closely with Community Services, Court Services, private providers, outside resources, Healthcare organizations, and programs at Canvas Health. Location Oakdale Schedule Monday through Friday, 40 hours/week Minimum Qualifications
