Schedule: Monday - Friday (8:00am - 5:00pm)

Compensation Range(s):



  • SUDS Counselor R (CADC-R): $23.36 - $30.37/hour
  • SUDS Counselor I: $25.76 - $33.49
  • SUDS Counselor II: $28.40 - $36.92



MINIMUM QUALIFICATIONS (REQUIREMENTS)

***Note: In order to be granted formal consideration for the SUDS Counselor position, please ensure your professional/working experience reflects the following requirements below:



  1. Required: High School Diploma or GED
  2. Required: The following CADC certifications are a prerequisite for employment...


    1. SUDS Counselor R requires a CADC R certification
    2. SUDS Counselor I requires CADC I
    3. SUDS Counselor II requires CADC II


  3. Required: Must have experience working in chemical dependency treatment, preferably with outpatient adults who have been adjudicated to treatment and/or people with co-occurring mental health disorders.
  4. Required: Must be able to demonstrate a working knowledge and skills in group facilitation.
  5. Required: Must be able to demonstrate working knowledge and skills in evidence-based treatment methods utilized in the treatment of addiction and trauma-informed care.
  6. Required: Knowledge and skills in de-escalation methods or ability to be trained in de-escalation methods.



PREFERRED QUALIFICATIONS



  1. Advanced degree



SUMMARY:

River Haven is an innovative program that will offer integrated housing, health care, and employment services with the long term goal of eliminating homelessness. We seek to change how these services are delivered to all people, especially vulnerable populations.

This position will work as a member of a multi-disciplinary team at River Haven to deliver comprehensive, person-centered, integrated services to the Portland Eastside community.

The SUDS Counselor works as a member of a treatment team providing substance abuse treatment and case management. The team works to address all treatment needs, including crisis management, substance abuse treatment, physical health management, mental health management, accessing affordable housing, accessing employment and educational opportunities. This position has access to mental health billing and service records.

ESSENTIAL DUTIES AND RESPONSIBILITIES:



  1. Conduct screening and assessment of potential clients to determine treatment needs, placement, and diagnosis; develop an individualized treatment plan to reflect these
  2. Participate in direct treatment of clients by:




    1. Attending clinical staff meeting and case conferences in order to facilitate individual treatment plans for clients;
    2. Providing group counseling to clients as needed.
    3. Provide case management to clients on an as-needed basis




  1. Conduct treatment groups and provide individual counseling as needed.
  2. Adhere to agency norms for completion of all clinical documentation, including but not limited to assessment, treatment plan, progress notes, monthly reports to referral sources, and service conclusion summaries.
  3. Participate in all Quality Assurance activities.
  4. Participate in clinical meetings.
  5. Work as a member of River Haven multi-disciplinary team to deliver comprehensive, client centered and trauma informed integrated services.
  6. Actively participate in the integration of services at River Haven and identify new opportunities to improve collaboration between and within service and care teams while carefully following information-sharing guidelines.
  7. Proactively identify opportunities for improvement within River Haven workflows and logistics. Participate in review of River Haven Key Performance Indicators; review and contribute to continuous improvement opportunities and plans.
  8. Build understanding of and appreciation for diversity among River Haven clients and support their connection to culturally responsive and culturally specific services as requested.
  9. Attend all mandatory CCC trainings in a timely manner.
  10. Document client services in electronic health records within 72 hours.
  11. Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC policies and agreements regarding confidentiality, privacy, and security. Support compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediately reporting any breach of protected health information or personal identification information of any person receiving CCC services by CCC or an outside provider to the CCC Compliance Department, as well as to your supervisor or their designee.
  12. Perform other duties as assigned.

  1. Supervise practicum students when needed.
