Now Hiring: Join Central City Concern's clinical leadership team as our Director of Nursing

Location: Central City Concern - Old Town Clinic (727 W. Burnside Street, Portland 97209)

Schedule: Monday - Friday (8:00am - 5:00pm)

MINIMUM REQUIREMENTS:



  1. Required: Bachelor's degree in Nursing or relevant field.
  2. Required: Unencumbered current Oregon Registered Nurse license.
  3. Required: A minimum 5 years of progressive, multi-site nursing leadership experience, including hiring, performance management, budget management, education and training is required.
  4. Required: Must have strong experience in an ambulatory setting work environment.
  5. Required: Must have reliable access to transportation for regular outreach activities. If using a vehicle for agency use: Must possess a current driver's license, access to a vehicle, qualify as an Acceptable Driver as designated in Central City Concern's Fleet Safety policy; pass an initial drivers training within 60 days of being an approved driver and continued recertification training; and maintain vehicle insurance coverage of a minimum of $100,000/$300,000 personal auto liability coverage.



PREFERRED QUALIFICATIONS ("GREAT TO HAVES")



  1. Master's degree in Nursing or relevant field.
  2. Bachelor's Degree in Nursing (BSN) and/or registered with MHACBO as a QMHP.
  3. Mental health/behavioral health nursing experience in a community health setting.
  4. Passion for building and developing nursing teams.



SUMMARY: This role is accountable for the development and oversight of CCC's nursing practice and clinical support functions by ensuring consistency in standards of practice across clinical settings at multiple sites such as OTC, OTRC, Blackburn Center, Hansen, Hooper DSC, Evergreen, LOC, and 16th & Burnside Recovery. The role will work in collaboration with clinical and operational leaders to align nursing strategy, clinical practice, operational processes, and financial performance in support of CCC's mission and vision. The scope of accountability for the Director role includes ambulatory primary care and specialty mental health clinic sites. The role will develop the relationships and organizational infrastructure to create a practice of nursing and clinical support functions across CCC.

ESSENTIAL DUTIES AND RESPONSIBILITIES:



  1. Provides a single point of accountability for nursing practice for the CCC organization-wide, acting as a liaison for outside agencies and regulators.
  2. Serves as a member of the Health Services Leadership Team and Senior Leadership Team, building and supporting collegial relationships to support client health in an integrated model.
  3. Develops comprehensive onboarding plans for nursing staff to ensure new hires receive orientation and training to our nursing standards and best practices. This includes competency assessment protocols.
  4. Develops and implements a standard methodology for competency, performance and productivity evaluation and tracking to ensure that clinical staff have the skills to perform their duties.
  5. Assures that nursing managers and supervisors monitor quality indicators and quality assurance checklists and take corrective action when improvement opportunities are identified.
  6. Collaborates with other disciplines for the implementation of programs, policies, and procedures that address how patient care needs of the patient population served are assessed, met and re-evaluated.
  7. Collaborates with providers, managers, staff and clients to develop, implement, communicate and improve clinical workflows.
  8. Collaborates with stakeholders to align and integrate workflows across programs and disciplines to facilitate client-centered care pathways.
  9. Collaborates with stakeholders to optimize and coordinate activities that support top-of-license, high quality team-based care related to onsite and virtual client visits.
  10. Develops population-based, proactive care management activities to improve disease prevention, management and monitoring and to address ambulatory-sensitive emergency department and hospital utilization.
  11. Promotes the use and implementation of technology in the workplace in order to streamline, monitor and optimize work processes.
  12. Develops and uses data to develop strategies to ensure that the care provided meets the changing needs of clients and payers.
  13. Coordinate clinical support functions for optimal service delivery (pharmacy, telehealth, nurse triage, care management, etc.).
  14. Manages nursing and clinical support staff leaders and provides performance feedback and goal setting; conducts annual performance assessments and skill development and training.
  15. Establishes mechanisms to assure nurses and clinical support staff throughout the organization are involved in decision-making structures and processes that establish standards of practice and address issues of concern.
  16. Collaborates with HR team to develop a talent acquisition plan for nurses and clinical staffing to recruit, select, orient and train new staff.
  17. Monitors and analyzes nursing trends, including those of scope of practice, as it pertains to CCC's core services and mission to end homelessness.
  18. Occasional RN shifts in various programs.
  19. Performs other duties as assigned.


SKILLS AND ABILITIES:



  1. Proven experience in broad agency-wide nursing practice oversight.
  2. Possesses strong sense of organizational dynamics, collaborative style, an adaptive thinker with high energy and enthusiasm, including ability to consult widely, bring together diverse views, and facilitate agreement on critical issues.
  3. Exceptional capacity for managing, leading, and developing clinical staff and leaders; a collaborative team builder who has experience in scaling organizations; ability to connect staff both on an individual level and in large groups; capacity to reinforce accountability.
  4. Quality decision making through a combination of analysis, wisdom, experience, and judgment.
  5. Knowledge of quality assurance/performance improvement, risk management, utilization management and outcomes management standards.
  6. Able to demonstrate high degree of independence, initiative and organization.
  7. Excellent verbal and written communication skills - able to communicate with a diverse population in a consistently informative, confident, respectful and professional manner.
  8. Able to demonstrate exceptional discretion, judgment, integrity and confidentiality when dealing with personnel issues, financial issues and intellectual property.
  9. Strong understanding of nursing implications in moving from a fee-for-service model to value-based model.
  10. Ability to create a work environment that encourages and fosters the growth of future leaders in the organization.
  11. Strong quality and process improvement, workflow development and change management experience.
  12. Ability to consider the impacts and outcomes for underserved communities during decision-making process.
  13. Ability to consider impacts of oppression, structural racism, and individual bias on client outcomes.
  14. Ability to utilize de-escalation techniques, when encountering volatile situations.
  15. Ability to work in an environment where people may be hostile, abusive or experiencing personal crisis.
  16. Physical ability to bend, stoop, kneel, squat, twist, reach, pull and lift heavy objects. Able to climb stairs several times a day.
