Overview

The Family Peer Support Specialist (FPSS) is responsible for providing support services and serving as a client advocate, while empowering families to find their own voice. The FPSS has the opportunity to positively impact families through support and education. The FPSS performs a wide range of tasks to assist families / clients in their own journey through the mental health system, role model good communication skills and share their own experineces in a positive manner. The FPSS will serve as a member of the Eyerly Ball team and provide support and consultation to encourage and model positive behaviors with clients and families in the CCBHS catchment area. Provide a variety of services, ranging from the teaching of skills to delivery of services to providing individual and group support focused on outreach and engagement. Will develop effective working relations and cooperate with all Eyerly Ball departments throughout the entire coordination process. This role is critical to ensure continuous improvement in client care, communication, and satisfaction.

Responsibilities

Care Coordination:

* Follow up monitoring

* Attending joint staffing treatment meetings

* Support coordination of care with primary care providers and specialist

Comprehensive transitional care:

* Engage clients and families as an alternative to emergency room or hospital care

* Participate in development of crisis plans

* Monitor for potential crisis escalation / need for intervention

* Follow up phone calls and face to face visits with client / families after discharge from the emergency room or hospital

Individual and Family support services:

* Help clients overcome barriers to services which include, but are not limited to anxiety, financial deficits, transportation issues, health limitations, etc.

* Providing assistance to clients in accessing needed self-help and peer/ family support services

* Advocacy for clients and families

* Family support services for clients and their families

* Assisting clients and families to identify and develop social support networks

* Support Medicaid adherence efforts, as needed

* Identifying community resources that will help clients and their families reduce barriers to their highest level of health and success

* Linkage and support for community resources, insurance assistance, waiver services

* Connection to peer advocacy groups, family supports networks, wellness center, NAMI, and family programs.


Qualifications

Education: High School Diploma required.

Experience: Experience as a parent or primary caregiver of a child with an emotional, behavioral or mental health need OR individual with experience with a friend or family member who has symptoms of a mental health condition. Experience navigating child serving mental and behavioral health systems.

License(s)/Certification(s): Have completed or will complete a state sanctioned Family Peer Support Specialist training and passed competency exam based on training. Possess a valid driver's license, and a good driving record.

Monitoring - Monitoring/Assessing performance of yourself, other individuals, or organizations to make improvements or take corrective action. Critical Thinking - Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions or approaches to problems. Active Listening - Giving full attention to what other people are saying, taking time to understand the points being made, asking questions as appropriate, and not interrupting at inappropriate times. Writing - Communicating effectively in writing as appropriate for the needs of the audience. Time Management - Managing one's own time and the time of others. Social Perceptiveness - Being aware of others' reactions and understanding why they react as they do. Customer and Personal Service - Knowledge of principles and processes for providing customer and personal services. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
