Position Highlights:

  • Position: BH Crisis Case Manager

  • Location: LOH

  • Full Time- 36/hrs week

  • Hours: 8:30am - 5:00pm

A Brief Overview:
Assesses the clinical needs of customers seeking behavioral health services and provides treatment referrals which are consistent with the customer's medical and psychiatric status/symptoms at Edward Hospital and Elmhurst Hospital. Assists in coordination of discharge planning services to patients who need ongoing behavioral health care following discharge. In collaboration with other members of the healthcare team, assesses behavioral health needs of the patient and offers suggestions regarding follow-up care options. Prepares and updates medical records including nature of discharge needs, resources available, patient and family response to recommendations and the final discharge plans arranged. Coordinates with or directs patients to designated community agencies or resources, as necessary. Mentors new BH Crisis Case Manager to the department. Leads departmental work groups in performance improvement activities or care conferences and huddles.

What you will do:

Responsible for conducting crisis clinical assessments for patients seeking treatment at Edward Hospital and Elmhurst Hospital

  • Appropriately assesses their clinical needs. Obtains initial diagnostic impression and confirmation during the disposition process with the physician or designee.

  • Assesses patients including vital signs, weight, blood alcohol and calibration of BMI for specific population.

  • Reviews previous documentation prior to assessment process and "case formulation".

  • Provides treatment referrals which are consistent with the customer's medical and psychiatric status and symptoms.

  • Identifies potential/actual problems that may adversely affect the patient and/or department. Reports situations immediately.

  • Oversees follow up with referral sources as appropriate.

  • Obtains collateral with families and support systems through face-to-face interviews and/or phone calls.

Consults with multidisciplinary treatment team members in all areas of the hospital on the assessment, treatment and discharge planning of patients with mental health problems.

  • Responds to physician orders and/or clinical staff requests to provide direction in the care of patients with mental health needs.
  • Collaborates with treatment team to design an appropriate plan of care.
  • Provides ongoing monitoring and follow through to assure implementations of stated recommendations.

Completes and acts as a resource to hospital staff on admission paperwork and patient rights.

Contacts and coordinates with families and community agencies as appropriate

Participates in care conferences and emergency department huddles to discuss key social, financial, and discharge planning factors which may affect patients' responses to treatment.

Demonstrates comprehensive understanding of hospital admission policies, including but not limited to EMTALA, Illinois Mental Health Code and Confidentiality Act and HIPAA.

Demonstrates understating of Behavioral Health Reimbursement systems, including managed care contracts, utilization management processes, Medicare and Medicaid.

Obtains pre-certification with insurance companies when necessary.

Documents precert information in Epic system and appropriately hands off to UM Dept.

Facilitates bed management planning and coordination with LOH nursing supervisors and LOH ARC Charges.

Functions as backup for Assessment Specialists when needed.

Educates patients and patient families regarding the effects of illness and disability on personal adjustment, family dynamics, relationships, social issues and the like.

Maintains accurate and timely medical records including initial assessment, progress notes, discharge dispositions, and so forth.

Assures all communication, verbal and written is clear, accurate and timely. Employee reflects feedback in their performance.

Provides support and guidance to new BH Crisis Case Managers during and after orientation.

Educates new employees in the department to clinical skills related to assessment, documentation and understanding processes and legalities of the department.

What you will need:

Required Education, Skills and/or Experience:

  • Master's Degree or higher in a behavioral health field

  • A high level of communication skills is required in order interact effectively with others in very difficult situations

  • Knowledge of DSM5 (Diagnostic and Statistical Manual of Mental Disorders) diagnostic criteria, ICD10 and interqual/MCG standards

  • A minimum of two years of previous assessment experience

Preferred Education, Skills and/or Experience:

  • Minimum of two years of experience in crisis/emergency services, behavioral health setting

  • Knowledge of patient rights and law pertaining to mental health

Required License and/or Certification:

  • Licensed Social Worker (LSW), Licensed Clinical Social Worker (LCSW), Licensed Profession Counselor (LPC), Licensed Clinical Professional Counselor (LCPC) licensure

  • Current State of Illinois driver's license required if the incumbent is selected to perform related duties at an alternate location. If the incumbent uses their personal vehicle, the incumbent must maintain automobile liability coverage as required by law and evidence of such coverage may be requested.

  • CPI certification is required within 90 days of start date into position.

  • Current CPR Certification issued by either American Heart Association or Red Cross within 90 days of start date into position.
