Position Summary

Resources For Human Development's Shared Living Program in Sioux Falls is HIRING a Team leader for the Level 2 and Level 3 homes, and Day Services. A Team Leader is responsible for coordinating comprehensive services for individuals their program supports. The Team Leader works closely with program leadership (directly supporting the Site Supervisor), as well as clinical and direct support professionals to ensure that each individual supported is empowered to live the life that they want to live.

The schedule for this position is as follows:

Week 1: Monday 10am-1pm lead and 1pm-6pm supports, Tuesday 10am-3:30pm lead and 3:3pm-6pm supports, Wednesday 8am-4pm Day Services, Thursday 8am-4pm Day Services, Friday 10am-3:30pm lead and 3:30pm-6pm supports

Week 2: Monday 10am-1pm-lead and 1pm-6pm supports, Tuesday 10am-3:30pm-lead and 3:30pm-6pm supports, Wednesday 8am-4pm Day Services, Thursday 8am-4pm Day Services, 10am-3:30pm lead and 3:30pm-6pm supports

A Team Leader provides oversight and supervision to staff and is responsible for coordinating comprehensive services for individuals their program supports. The Team Leader works closely with program leadership, as well as clinical and direct support professionals to ensure that each individual supported is empowered to live the life that they want to live. A commitment to the RHD values should be demonstrated as job duties are performed.

Essential Duties and Functions

Staff Supervision

  • Provides first-line supervision of direct care staff within their program through coaching, mentoring and supporting their performance and professional development.
  • Organizes, schedules and distributes work among assigned personnel.
  • Keeps personnel informed of pertinent policies and procedures affecting the program and/or their jobs.
  • Administers policies and procedures as established by Resources for Human Development

Direct Care

  • Maintain direct service case load.
  • Coordination and oversight of medical appointments, including attending appointments if necessary. Communication with nurses and doctors about consumers' medical needs as appropriate.
  • Maintains positive communication and relationships with family members, guardians, staff, and Regional Office personnel about policies, issues, and/or concerns.

Administration

  • Actively participates in the person-centered planning process, including but not limited to:
  • attending all planning meetings,
  • providing input,
  • coordinating the development of training plans,
  • ensuring all necessary documentation is completed,
  • reviewing progress notes and daily notes,
  • monitoring for the implementation of the plan,
  • completing monthly reviews, and
  • communication with service coordinators.
  • Prepare and submit applicable reports and documentation related to individuals supported by their program.
  • Ensure compliance with all regulatory bodies.
  • Implement and complete continuous quality improvement activities.
  • Other duties as assigned

Qualifications

  • Bachelor's Degree in a Human Services field, including but not limited to sociology, special education, rehabilitation, nursing, counseling, and psychology OR
  • An associate degree with three (3) years' experience working to support individuals with intellectual disabilities OR
  • A High School Diploma with five (5) years' experience.
  • Current Certified Psychiatric Rehabilitation Practitioner (CPRP) Certification
  • Valid Driver's License
