Position Summary

The LPN is responsible for overseeing all aspects of medical care for those individuals RHD provides services.

Essential Duties and Functions

  • Act as part of a multidisciplinary team designed to provide services from a holistic and Values based perspective
  • Manage the ordering/delivery/storage/ administration of medications for the persons served in accordance with the specific program/service
  • Provide education to both persons served and staff regarding common health concerns. Provide training to staff as required/requested in order to best meet the needs of persons served.
  • Assess and respond to new/potential/ ongoing health issues in persons served.
  • Complete documentation as required by RHD, funder/licensing/accreditation bodies.
  • Follow agency, MCO, County, and licensing regulations.
  • Supervise consumers to ensure safety and health of all residents.
  • Follow agency protocols for assessment, documentation, and reporting client's issues and status.
  • Provide nursing care such as wound care, diabetic counseling, and etc, as needed.
  • Provide primary medical care to individuals needing detoxification services
  • Provide daily medication monitoring and medical services to all residents in the program in addition to individual consultation and programming to support the resident's overall wellness
  • Build relationships with the program's contracted pharmacy and outside medical providers and assist the women with connecting to primary care providers and other needed medical services and medication assisted treatment in the community
    • Additional duties include:
      • Supervising client submission of urine for analysis
      • Individual dialogue and intervention about health issues
      • Participation in weekly clinical rounds and case consultations
      • Tracking and ordering medication refills and completing documentation in a SOAP note format
  • The nurse will be responsible for making independent decisions and problem solving medical challenges with limited direction from others during their shift.

Qualifications

  • LPN
  • Valid Driver's license
  • Act 33/34 clearance and FBI clearance as needed
  • 3 years experience in a drug and alcohol detoxification setting or with individuals on MAT preferred
  • Preference given to individuals with experience as a community nurse outside of a hospital setting.

Appropriate candidates will be compassionate, caring and enthusiastic about our mission, and will possess the desire to interact with individuals creatively through modeling, teaching, assisting, and listening. Willingness to support the development of this pilot project is needed and ability to support the trauma informed culture and interact with everyone in a respectful and gentle way is a must. Additionally, willingness to work with individuals on medication assisted treatment and support women in all stages of recovery is required. Bilingual (English/Spanish) are strongly encouraged to apply.

COVID VACCINE MANDATE:

RHD employees and programs in the City of Philadelphia are required to follow the COVID vaccine mandate. Revised PDPH guidance allows for new hires to be partially vaccinated at the time of hire. This means they have received at least one shot in a two-dose series or a single dose in a one-dose series before beginning in-person shifts. The final dose must be received within 8 weeks of hire (start date). This currently does not include boosters.

RHD's vaccine requirements for the Corporate Office located at 4700 Wissahickon Avenue; Suite 126, Building A. Philadelphia, PA:

The Corporate Office is not a service location. Therefore, RHD has determined that vaccinations are not required to work out of or to visit the office. As a reminder and as stated in prior communication, vaccinations are required for those who visit or work in Philadelphia programs and/or service locations.

Salary Description$50.06 per hour
