Description

JOB PURPOSE:

Provide oversight of case management operations and ensure compliance with VA Supportive Services for Veteran Families regulations, grant requirements, and policies. Supports the implementation, operation and evaluation of the SSVF program, serving Veterans and their families who are homeless or at risk of homelessness.

Qualifications

ESSENTIAL JOB RESPONSIBILITIES:



  • Meet regularly with Case Managers to staff caseloads and keep abreast of current issues. Keep Case Managers informed of program information. Provide Quality Assurance and regulatory compliance to client files. Provide on-going program evaluation and recommendations to the Program Director for continuous growth and quality. Prepare and submit monthly program reports. Produce ad hoc reports as requested by Program Director.
  • Manage office coordination, client flow, Veteran complaints and Crisis Management.
  • Advocate for and facilitate Veterans' access to community resources; housing assistance, utility assistance, and relevant community services and benefits. Build database of community resources. Provide referrals to community resources as needed.
  • Ensure all participants files meet VA SSVF eligibility, enrollment, recertification, and discharge requirements.
  • Conduct regular case file audits to verify complete and accurate documentation, including assessments, housing stability plans, case notes, and supportive service records. Track corrective actions and implement quality improvements initiatives.
  • Ensure HMIS data quality, complete, and timely entry of services and case notes.
  • Maintain strong working relationships with VA Medical Centers, HUD-VASH teams, Continuum of Care partners, landlords, and community agencies.
  • Be active in and network at monthly Veteran community groups, and case conferencing. Provide presentations at various community groups within the service area. Promote the success and reputation of the Supportive Services for Veteran Families program.
  • Other duties as assigned.

ESSENTIAL QUALIFICATIONS:

EDUCATION: Bachelor's Degree in Social Work, Sociology, Psychology or related field.

EXPERIENCE: 1+ years of case management experience; 3+ years preferred. 2+ years of supervisory experience; case management supervision preferred. 2+ years in a customer service focused environment. Experience with homeless, low income, veterans & their families a strong plus.

ATTENDANCE: Must maintain regular and acceptable attendance at such level as is determined in the employer's sole discretion.

LICENSES: LMSW, LBSW, LMFT preferred. Driver's License with clear record.

VEHICLE: Must have daily use of a vehicle without prior notice.

OTHER: Cross train into Program Director position responsibilities. Must be available and willing to travel to various locations and with such frequency as the business need dictates. Must be available and willing to work nights, weekends and holidays as required to meet business needs. Must not pose a direct threat or significant risk of substantial harm to the safety or health of himself/herself or others.
