Position Summary Information
| Primary Purpose |
The Registered Nurse / Clinical Case Manager provides direct clinical nursing care to students while also serving as the coordinating clinician for students with complex, chronic, or high-risk health and behavioral health needs. This dual role blends hands-on nursing practice with longitudinal case management-assessing needs, developing care plans, coordinating services across campus and community resources, and ensuring continuity of care. The position supports the university's mission of promoting student health, academic success, retention, and wellbeing.

| Essential Functions |
- Perform nursing assessments, triage (in person, by phone, and via telehealth), and evaluate acute and chronic health concerns for CAPS and SHS
- Administer medications, immunizations, injections, perform phlebotomy, and treatments per provider orders and standing orders/protocols.
- Assist providers with examinations and procedures; perform point-of-care testing, wound care, and specimen collection
- Provide health education and counseling on topics such as sexual health, mental health, nutrition, substance use, chronic disease self-management, and preventive care.
- Respond to medical emergencies on campus and initiate appropriate interventions and referrals.
- Document care accurately and timely in the electronic health record (EHR) in compliance with regulatory and professional standards.
- Support campus public health initiatives, including communicable disease surveillance, outbreak response, and immunization compliance.
- Attends and contributes to the department staff meetings.
- Assists staff in preparing outreach/workshop materials.
- Performs other duties as assigned or requested.
Clinical Case Management
- Identify students with complex medical, mental health, or psychosocial needs.
- Conduct comprehensive biopsychosocial assessments and develop, implement, and monitor individualized care plans with measurable goals.
- Coordinate care across wellness departments (counseling and psychological services, services for students with disabilities, dean of students, and external providers (specialists, hospitals, community agencies, insurers).
- Facilitate care transitions, including hospital discharge follow-up, return-to-campus planning, and referrals to higher levels of care.
This position serves as Campus Security Authority (
CSA) as outlined by the Clery Act. CSAs have federally mandated responsibilities to report crimes that are reported them.

| Minimum Qualifications |
- Current, unencumbered Registered Nurse (RN) license in California
- Bachelor of Science in Nursing (BSN) [or Associate degree in Nursing with relevant experience
- Current BLS certification.
- Up-to-date titers and Immunizations
- Minimum of three (3) years of clinical nursing experience; ambulatory, community, public health, inpatient.
- Minimum of two (2) years of experience in a clinical case management role
- Demonstrated knowledge of case management principles, care coordination, and community resources.
- Proficiency with electronic health records and standard office technology.
- Strong assessment, triage, communication, organizational, and documentation skills.
- Ability to work independently, manage a caseload, and exercise sound clinical judgment.

| Preferred Qualifications |
- Five (5) or more years of clinical experience in ambulatory, community, public health, or inpatient settings.
- Four (4) or more years of experience in a clinical case management role.
- Case Management certification (e.g., ACM-RN, CMGT-BC)
- Experience in a college/university health, adolescent/young adult, or behavioral health setting.
- Knowledge of FERPA and higher-education health service operations.
- Cultural competence and experience serving diverse student populations
- Experience and sensitivity in working with people of diverse backgrounds and cultures.
- Demonstrated experience in advancing social justice, equity, and inclusion in a university setting.
- Ability to engage and integrate culturally responsive practices and knowledge in their work.

- Valid driver's license is required. Incumbent must also be able to meet the University's fleet rules and be eligible to drive for University business. The University and its insurance carrier reserves the right to exclude applicants based on their driving record.
This is an in-person role, and the successful candidate must be willing to relocate within California, preferably within 25 miles of the San Francisco Pacific campus. |
| Hiring Range |
$52.00 per hour. We consider factors such as, but not limited to, scope and responsibilities of the position, candidate's qualifications, internal equity, as well as market and organizational considerations when extending an offer. |
