DESCRIPTION OF WORK

This position focuses on assessing risk and creating care plans that reduce or remove danger for older adults who are reported in need. The role requires careful evaluation, clear documentation, and steady coordination with various supports. As an Aging Care Manager 3, you will perform the following duties:
- Risk Assessment: Gather information from multiple sources to evaluate the level of danger facing the consumer
- Report Determination: Review facts and determine whether the report of need is substantiated or unsubstantiated
- Care Planning: Develop plans that address presenting and underlying problems through formal and informal supports
- Case Documentation: Maintain complete and timely records of investigative activities, observations, and contacts in the case system
- Monitoring Visits: Conduct home visits and phone contacts to review behavior, verify safety, and assess care plan effectiveness
- Emergency Response: Arrange urgent shelter, initiate necessary involuntary procedures, and support court actions when risk cannot be reduced through other measures
Work Schedule and Additional Information:
- Full-time employment
- Work hours are 8:30 AM to 4:30 PM, Monday - Friday, with 30-minute lunch.
- This position requires frequent travel for client visits and court hearings.
- Telework: You may have the opportunity to work from home (telework) part-time after six months, depending on operational needs. In order to telework, you must have a securely configured high-speed internet connection and work from an approved location inside Pennsylvania. If you are unable to telework, you will have the option to report to the headquarters office in Eddystone. The ability to telework is subject to change at any time. Additional details may be provided during the interview.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS Minimum Experience and Training Requirements:
- One year as an Aging Care Manager 2 or Aging Program Assessor; or
- One and one-half years of professional human services, law enforcement, or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training which includes six months of professional human services, law enforcement, or nursing experience.
Other Requirements:
- This particular position also requires possession of a valid driver's license which is not under suspension.

- You must be able to perform essential job functions.
Legal Requirements:
- You must pass a background investigation.
- This position falls under the provisions of the Older Adult Protective Services Act.
- Under the Act, a conditional offer of employment will require submission and approval of satisfactory criminal history reports, including but not limited to, PA State Police and FBI clearance.
