What You'll Do
As an Outreach Case Manager based in West Bend, you will be a vital link between individuals in crisis and the services, resources, and community connections they need to thrive. Our Outreach & Case Management Program is free of charge to clients. It helps people prioritize their needs, create sustainable plans, and move toward long-term stability and self-sufficiency. You will work independently in the community while collaborating with a supportive, mission-driven team and community partners.

• Develop Individualized Care Plans: Complete client needs assessments and create personalized care plans that address financial, physical, and emotional support.
• Connect Clients to Essential Services: Help clients enroll in programs such as Medicare, food assistance, disability benefits, and other community resources.
• Advocate During Times of Crisis: Connect clients to the right resources when they need them most and provide ongoing personal support.
• Monitor Progress and Outcomes: Track client progress and adjust care plans to ensure positive, sustainable results.
• Maintain Accurate Case Records: Document significant events and client progress accurately and on time.
• Collaborate for Comprehensive Care: Partner with internal programs and external community organizations so clients receive every service they need.

Salary & Benefits

$45,000-$47,000, depending on experience.

Qualifications

• Bachelor's degree in social work or a related field strongly preferred.
• Two years of experience in case management, family services, or social services preferred.
• Bilingual proficiency in English and Spanish strongly preferred.
• Travel within the community required.
• Valid driver's license, reliable transportation, and vehicle liability insurance of $100,000 per person / $300,000 per accident required.
