In this role, you'll have the flexibility to create a schedule that works for you while making a difference in the lives of patients. Providers can flex their hours between 8:00 AM and 8:00 PM throughout the week, with no on-call responsibilities, no weekends, and no holidays required.

About HouseCalls

HouseCalls is an innovative, outcomes-focused program designed to deliver preventive care where patients are often most comfortable, in their own homes. As an Advanced Practice Clinician (Nurse Practitioner), you'll conduct annual in-home health assessments for Medicare Advantage and other health plan members.

This is a non-prescribing, field-based role that focuses on health education, identifying potential care needs, addressing gaps in care, and collaborating with primary care providers to support each member's overall health journey. You'll spend more time connecting with patients and helping them navigate their health concerns while enjoying the independence and flexibility that comes with a mobile care model.

Primary Responsibilities
  • Conduct comprehensive in-home health assessments, including:
    • Medical history review
    • Medication reconciliation
    • Vital signs and physical examinations
    • Evidence-based screenings and point-of-care testing, as appropriate
  • Identify and document clinical conditions to support care planning and coordination
  • Communicate assessment findings to primary care providers to help address gaps in care
  • Recognize urgent or emergent health concerns and take appropriate action
  • Educate members on disease management, medications, and treatment adherence
  • Assess social determinants of health and connect members with community resources when needed
