Description Schedule Details: Full Time, Evening Shift - 12-hour shifts every Sunday through Tuesday + 8 hour shifts every other Wednesday Responsible for the coordination of care and mental health referral services for Covenant Health emergency departments, providing interventions for patients. Provides referral and connection to needed services for patients and families presenting to the emergency departments, working to prevent avoidable admissions/readmissions. Provides assessment and appropriate interventions for patient care needs. Works to ensure appropriate utilization of acute care beds through assessment of alternative site of care options. Collaborates with medical care teams, community agencies and patients/families to ensure safe discharge planning. Works to determine alternative site of care options and initiates proactive discharge planning for patients being admitted. Assists multidisciplinary team to recognize and respond to mental health needs through assessment and disposition. Responsible for entering into the database/tracking and all EMR documentation of services provided. Providence caregivers are not simply valued - they're invaluable. The successful candidate's placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
