Children's Health Home Care Manager (5277)

• Develop, document, and maintain individualized, family-centered plans of care that integrate medical, behavioral health, rehabilitative, long-term care, and social-service needs.
• Coordinate services across primary care providers, specialists, behavioral health professionals, schools, community agencies, and other partners involved in the child's care.
• Help children and families access preventive care, wellness services, medical treatment, behavioral health support, and community resources.
• Build relationships with children and families that encourage continued engagement in care and support adherence to treatment recommendations.
• Monitor each child's needs, progress, care transitions, and potential gaps in services.
• Coordinate post-discharge follow-up, including obtaining discharge records, supporting medication reconciliation, scheduling recommended appointments, confirming attendance, and re-engaging families when appointments are missed.
• Maintain accurate and timely health records in NetSmart so the multidisciplinary care team has access to current information and care plans.
• Identify available community resources and actively manage referrals, access, follow-up, and coordination of services.
• Advocate for children and families as they navigate complex healthcare and social-service systems.
• Perform other duties as assigned.

Candidates must meet one of the following education and experience combinations:

• Bachelor's degree with at least two years of experience working with children and families.
• Master's degree with at least one year of experience working with children and families.
• Current Registered Nurse license with at least two years of experience working with children and families.

Experience in child welfare is strongly preferred.

Additional requirements include:

• Valid driver's license in good standing, reliable transportation, and appropriate automobile insurance.
• Ability to travel regularly for visits and appointments, including weekly travel of up to one hour from the Albany office.
• Ability to organize travel efficiently across the assigned service area.
• Ability to work respectfully and effectively with children and families from a broad range of cultural, economic, and geographic backgrounds.
• Strong written, verbal, and active-listening skills.
• Ability to coordinate with multiple providers while maintaining accurate and timely documentation.

$22.12-$24.04 per hour, commensurate with experience.
