Role overview

The Care Manager will serve as an integral part of the agency's Care Management Services team, helping ensure older adults, their families, and caregivers have comprehensive, direct access to CJE SeniorLife's continuum of community-based services. The position's objective is to promote the well-being of older adults in the community so that they can remain independent for as long as possible, in line with the agency's mission. The Care Manager coordinates and links care across agency- and community-based health and social services organizations and provides client advocacy and support. The role involves maintaining a caseload of ongoing clients to provide support, guidance, referrals, and direct services.

Essential responsibilities




  • Screen, assess, and document the needs of new and returning clients. Facilitate internal referrals from agency colleagues and initiate and coordinate referrals to community-based resources.



  • Document cases in a timely manner including initial assessment, changes in goals and services, clinical case notes, and reassessment at specified intervals.



  • Uphold policies and procedures as stated in agency manuals and the clinical standards as related to the client journey.



  • Maintain a caseload of approximately 35 ongoing clients, completing in-home initial assessments (e.g., health, financial, psychosocial, cognition, etc.) with older adults and care partners to determine needs.



  • Develop, implement and maintain appropriate client care plans and service delivery, including providing in-person, in-home assistance as needed.



  • Utilize a trauma-informed person-centered approach for healthcare management, provide options and advocacy, maximize resources, and coordinate care.



  • Assist older adults and their support systems with the aim of enhancing the coordination of care and eliminating the duplication of services.



  • Advocate for the older adult both in the community and, as needed, during transitions between hospital/rehab/home.



  • Participate in individual and group supervision/consultation to enhance the quality of practice.



  • Work in an autonomous manner; when appropriate promptly seek supervision and consultation to address at-risk situations.



  • Participate in program planning and development.



  • Based on need, participate in outreach activities to generate referrals for care management.



  • Provide efficient and effective services to clients which meet Agency's Standards.



  • Other duties as assigned.




Compensation

$60,000 per year.

Schedule

Monday to Friday, 8:30 am to 5:00 pm.

Union status

This position has the option of becoming a dues-paying member of Service Employees International Union Local 73.

Experience and/or Experience




  • Graduate of an accredited four-year college or university with a degree in gerontology, health, social work or other related area. Minimum of two (2) years of experience in a human service field, or



  • Graduate of a master's level professional program with a degree in social work or related field (e.g., counseling, gerontology).




Certificates, Licenses, Registrations



  • License and master's degree preferred. Licensed professionals (e.g., L.S.W. or L.C.S.W.) are expected to maintain professional licensure and standards, including completion of CEUs.
  • Required to work toward Care Manager Certified (CMC) credential with support and training provided. This credential requires an associate's, bachelor's, or master's degree in social work or related field, plus 2 years direct practice with 50 hours supervision.
