Clinical Social Worker 3

The Clinical Social Worker 3 (CSW 3) primary responsibilities are psychosocial assessment, crisis intervention, counseling, brief therapeutic interventions, consultation, education, community linkages, supportive services, and to facilitate the plan of care through the health care continuum for patients, families, and staff, with particular focus on high-risk populations. The CSW 3 acts as a consultant to the clinical team, service lines and other departments and participates in program development and quality improvement initiatives. The CSW 3 will conduct legal and ethical billing for services when applicable and appropriate. The CSW 3 will primarily practice at the McClymond's High School FQHC Health Center location and at the Claremont Primary Care Clinic but also be available to the Castlemont High School FQHC Health Clinic, Claremont TEEN Adolescent FQHC site, FQHC Mobile Clinic as needed.

The CSW 3 will support students and community members in and around McClymonds High School as well as patients in the Encore Medical Clinic, a specialty clinic within Claremont Primary Care, which services unstably housed children and their families. The CSW 3 will participate in department education and practice improvement activities. The CSW 3 will provide social work services to any patient regardless of gender, gender identity, sexual orientation, social, cultural, educational or religious background, language, age, ancestry, citizenship, color, national origin, race, presence of mental or physical disability, lack of source of payment for care, medical condition, marital status or status as a veteran.

Department Description:

At UCSF Benioff Children's Hospital, we understand that navigating healthcare, hospitalization, illness or injury can be extremely stressful. Clinical Social Workers are available to help patients and their families cope with a broad range of psychological stressors related to illness and being in the medical environment. Our Social Workers are part of the health care team to support patients through the continuum of care. We strive to reduce health disparities to increase access to care through a trauma informed culturally appropriate approach.

DUTIES & ESSENTIAL JOB FUNCTIONS

1. Provide psychosocial assessment, crisis intervention, counseling, information, referral and linkage to community referrals for patients, their families and staff.

2. Active member of the interdisciplinary team, specifically on complex psychosocial cases, contributing to team goal setting and plan of care.

3. Inpatient CSW 3 develops and implements a safe discharge plan while the outpatient CSW 3 facilitates continued care planning. Focus for both is to address the biopsychosocial needs of the patient and their family while in collaboration with the interdisciplinary team members.

4. Assess, intervene with and report domestic violence, child/elder abuse and other reportable situations in accordance with legal mandates and UCSF policy and procedures.

5. Screen for suicidal/homicidal ideation and follow legal mandates and UCSF policy and procedures to assure patient safety. Collaborate with UCSF Psychiatry, the BERT team and other appropriate teams on cases meeting criteria for psychiatric holds or complex behavioral health concerns.

6. Consult on challenging custody issues to clarify decision-making capacity, identify surrogate decision maker(s), facilitate completion of Advance Health Care Directive and/or POLST; when appropriate coordinate application for probate conservatorship or court order.

7. Knowledge and utilization of the Diagnostic and Statistical Manual (DSM).

8. Knowledge and skill with substance use and dependence assessment and intervention strategies, mental illness, end of life/bereavement, benefits and entitlements, and adjustment to illness.

9. Complies with Department standards for timely submission, accuracy and completeness of documentation in the Medical Record and collects all required statistical data.

10. Conduct billing practices for encounters of patients' families, and members of the patient treatment team, as applicable to maintain productivity standard.

11. Coordinate and facilitate patient and family meetings with the interdisciplinary team.

12. Attends department, hospital and community in-services related to field of practice.

13. May facilitate department, hospital and community in-services related to field of practice.

14. Participate in quality improvement activities and program planning for the Department, Hospital and Service Lines.

15. As available and under the direction of the Supervisor, CSW 3 may provide clinical supervision to unlicensed Clinical Social Worker staff/interns for the purpose of LCSW licensure in compliance with the California Board of Behavioral Sciences (BBS).

16. Participates in coverage as needed and assigned.

17. Compliant with all UCSF policies relevant to scope of work.

Required Qualifications:

• Master's degree from a CSWE accredited school of Social Work.
• Current California State License of Clinical Social Work in good standing (CSW 3).
• Experience (internship/career) in a healthcare, mental health, and/or child protection working with children and families.
• Experience in crisis de-escalation and trauma informed care.
• Knowledge and abilities essential to the successful performance of the duties assigned to the position.
• Demonstrates resourcefulness, effective written and oral communication, diplomacy, and organizational and analytic skills.
• Self-directed, assertive and creative in problem solving, systems planning and patient care management in a high-volume work environment.
• Ability to work effectively and collaboratively with interdisciplinary teams.
• Proficient text/pager, telephone, and computer skills including the use of the internet.
• This position requires flexibility to orient and work at all UCSF Medical Center locations.

Preferred Qualifications:

• Experience working with pediatric patients, preferred.
• Second language, Spanish.

Licenses and Certifications:

• Current California State License of Clinical Social Work in good standing (CSW 3).
