Our Lee Physician Group (LPG) Community Care Outreach team is growing and seeking a compassionate, collaborative, and resourceful Master's Level Social Worker (MSW) to join our mission-driven team.

Meet Your Team

You'll be joining an incredible multidisciplinary team consisting of:

6 Master's Level Social Workers
5 Nurse Navigators

Together, we work to connect patients with resources, remove barriers to care, and improve overall health outcomes throughout our community.

What You'll Be Doing

Serve as a key resource for patients with social, financial, and healthcare-related challenges

Visit 45 primary care clinics throughout the community approximately 12 days per week

Collaborate with providers, nurse navigators, care teams, and community partners

Assess patient needs and connect them with appropriate community resources

Help patients navigate healthcare systems, insurance challenges, and social service programs

Participate in team meetings and case discussions

Utilize technology and documentation systems to coordinate patient care effectively

Advocate for vulnerable and underserved patient populations

Required Qualifications Education

Master's Degree in Social Work (MSW) Required

Experience

Minimum of 1 year of clinical experience in a medical, healthcare, or clinical setting

Additional Requirements

Reliable transportation required

Ability to travel to clinics, community meetings, and patient-related visits as needed

Comfortable working independently while remaining highly collaborative with the team

Strong computer and technology skills

Excellent communication and relationship-building abilities

Bonus Points If You Have

Bilingual skills (Spanish preferred)

Experience working with:

  • Uninsured populations

  • Underinsured populations

  • Socially vulnerable patient populations

  • Community outreach programs

Knowledge of community resources, assistance programs, and healthcare navigation

Experience coordinating care across multiple disciplines

The Social Work practice that occurs in hospitals and other health care settings inspires health, and aids physically ill patients and their families. The Social Worker assists in resolving the social and psychological problems related to the illness. The Social worker sensitizes other health care providers about the social and psychological aspects of illness. The Social Worker empowers patients to be proactive with regard to their health and personal well-being.

The Social Worker provides clinical coordination and therapeutic interventions through assessment, referral and interdisciplinary planning for high-risk populations. The Social Worker will link patients and families to systems that provide resources, services and opportunities to achieve and maintain maximum functioning. The Social Worker has the opportunity to provide Field Instruction for MSW students. The Social Worker provides counseling and support to patients and families regarding adjustments to disease process and illnesses. The Social Worker also expedites linking patients with systems in the community to provide them with resources, services and opportunities to achieve and maintain maximum functioning, and conduct screenings for CBT resource linkage.

Requirements

Education:

Masters Degree in Social Work required.

Experience:

Minimum of 1-year clinical experience in medical or healthcare setting.
