We are seeking a compassionate and experienced Registered Nurse to primarily support Hepatobiliary (HPB) Surgical team in a unique Triage & Care Coordinator role. In this position, you'll be at the heart of patient care providing clinical guidance, answering questions, coordinating next steps, and helping patients feel supported every step of the way.

This role blends triage, care coordination, and patient navigation, making it ideal for a nurse who enjoys meaningful patient interaction and thrives in a dynamic, team-focused environment.

Employment Type: Full-Time; 1.0 FTE (Monday-Friday)
Location:East Cancer Clinic (Clackamas, OR)

Salary Range: $41.17 - $69.36 Per Hour

**Final offer amounts are determined by a review of the candidate's applicable experience and qualifications, as well as, internal employee equity considerations.

Scope:

Serve as a trusted clinical resource for patients through telephone triage

Assess symptoms, provide guidance, and escalate care when needed

Partner closely with physicians and care teams to ensure timely, coordinated care

Help patients navigate their treatment journey from questions to follow-up

Build meaningful connections through compassionate communication and support

Document care accurately and maintain continuity across the team


Responsibilities

This is a full-time (1.0 FTE) position primarily supporting our HPB program. As part of a collaborative outpatient surgical services team, you'll also have the opportunity to:

  • The RN will support additional surgical specialties (e.g., breast, gynecologic surgery, and others as needed)
  • Cross training is expected to ensure coverage across service lines
  • The role requires flexibility, adaptability, and a team-oriented mindset within an outpatient setting
  • Addresses patient calls, portal messages and emails in a timely manner concerning a wide range of symptom/side effect related topics.
  • Asks appropriate, open-ended questions as well as utilizes computerized algorithms/pathways to assist in the assessment of patient's symptom and side effect management inquiries and documentation of such interventions.
  • Assesses urgency of patient symptoms / side effects and addresses them appropriately, aggressively managing them to prevent unnecessary emergency department and hospital utilization.
  • Documents all conversations with patients to maintain a comprehensive medical record.
  • Provides clear instruction and education to patients via the telephone.
  • Evaluates and determines if emergency care is needed, or if the patient can be seen in the outpatient clinic.
  • Collaborates with physicians via electronic communication, face to face, or the telephone to discuss patient care needs that cannot be independently and appropriately addressed using standing orders or practice protocols.
  • Sets up same day and/or future provider/infusion room appointments to address symptom management concerns.
  • Additional duties as assigned.

Qualifications

What You Bring

  • Active RN license
  • Graduate from an accredited program for professional nursing education, BSN preferred
  • Strong triage and clinical assessment skills
  • Experience in oncology, surgical care, or care coordination (preferred) or (Experience in triage, care coordination, or patient navigation strongly preferred)
  • OCN certification (or interest in obtaining) is a plus
  • A compassionate, patient-first mindset and strong communication skills
  • Current CPR certification required
