Job Description

Elwyn seeks a Coordinator of Performance Based Quality who ensures the development and ongoing implementation of Performance Based Contracting (PBC) and related processes in the pertinent Intellectual and Developmental Disabilities (IDD) programs (Residential Habilitation, Life Sharing and/or Supported Living Services in the Consolidated and/or 1915(c) Home and Community Based Waivers), to ensure compliance with the applicable t T ier designation(s) performance standards requirements through oversight, analysis, and potential realignment of related processes. This role develops and coordinates all PBC processes and training related to procedures involved in PBC and measures for IDD employees. The Coordinator of Performance Based Quality helps to design, implement and oversee related Electronic Health Record (EHR) processes in IDD programs.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Develop, integrate and implement a PBC management program, including design, plan, data collection, analysis, and reporting on the program's PBC measures for Elwyn's Community Residential Services (CRS) programs and other programs where PBC measures may be implemented
  • Ensure ongoing updating of the PBC management program upon new information from the Pennsylvania Office of Developmental Program's (ODP) related to residential performance standards and minimum thresholds as per ODP Bulletin 00-24-01 and any subsequent correspondence related to Performance Based Contracting or Pay for Performance (P4P) metrics
  • Obtain and maintain familiarity with all processes related to PBC, including but not limited to the Quality Management Plan (QMP), Risk Management (RM), Supports Intensity Scale (SIS), Assistive Technology Professional certificates and other staff credentialing, and Health Risk Screening Tool (HRST) to ensure that processes are in place to support PBC performance
  • Implement, oversee, and report on systems and activities related to PBC as critical key performance indicators (KPIs) during Continuous Quality Improvement meetings and as needed
  • Perform program integrity reviews and, audit, collect data regarding performance indicators, and conduct internal monitoring to ensure program and regulatory compliance in relation to PBC
  • Collaborate with operations, support departments and Quality Improvement personnel on all aspects relating to PBC or P4P
  • Develop policies and procedures related to PBC and program objectives, including but not limited to participation in the development of standard operating procedures, trainings, and other quality enhancing protocols and processes to ensure compliance with Elwyn policies and regulatory requirements (e.g. the Health Insurance Portability and Accountability Act (HIPAA), recordkeeping, etc.)
  • Develop and deliver training for all personnel regarding PBC, including related processes connected to the EHR
  • Communicate on an on-going basis with the directors regarding findings and other issues important to the performance on the PBC-related plans and areas in their respective programs, including standard performance reports to the Executive Director
  • Develop and maintain a thorough understanding of the service area's core competencies, current strengths and challenges, and assist the directors in developing programmatic enhancements, changes, and strategies to meet the needs of performance management under PBC and P4P
  • Develop and maintain PBC performance and measurement indicators, evaluate the usefulness of these indicators, and recommend corrective action plans for noted operating problems
  • Obtain and maintain ODP's Quality Management Certification to ensure program performance

QUALIFICATIONS

  • Master's Degree in a human service-related field preferred
  • Five (5) years' experience in residential service for people with disabilities management including but not limited to quality improvement, program administration and/or direct program operations supervision; experience working with people with intellectual and/or developmental disabilities preferred
  • Experience with implementation and application of EHR such as Netsmart/myAvatar or Therap with IDD programs
  • Knowledge in applicable regulations including but not limited to Pennsylvania regulations 6100, 6400, 6500, Home and Community waiver regulations
  • Obtain and maintain ODP's Quality Management Certification
  • Demonstrated ability to work effectively as part of a team including leadership ability
  • Must be organized with excellent attention to detail
  • Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
  • Demonstrated strong attention to detail
  • Demonstrated strong time management and organizational skills
  • Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision
  • Demonstrated ability to anticipate future needed changes or identify problem areas and take effective actions
  • Must possess excellent customer interaction, collaboration, presentation, written and verbal communication skills
  • Demonstrated intermediate experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint; OneDrive/SharePoint/Teams
  • Must have a current, valid driver's license in state of residence, two (2) years driving experience, and an acceptable driving record
