SUMMARY:

The Stroke Program Nurse Navigator is a registered nurse who reports to the Director of Emergency Services and is responsible for supporting clinical teams throughout the hospital who provide direct care to stroke patients, and their families. Patient navigation refers to the assistance offered to patients and their families to help them move through the complexities of the healthcare system and overcome any barriers to quality care. The Stroke Program Navigator recognizes that every patient and every stroke is unique, and that care must be individualized. The candidate will work closely with the Stroke Program Manager and function as a point person for patients and families during and following their hospitalization.

RESPONSIBILITIES:

Assumes key role in assuring care on a continuum, coordinating diverse resources and patient care activities for individuals receiving patient-centered stroke care. Acts as the point person for patients and families/caregivers throughout their hospital stay and following their hospital discharge. Key responsibilities include:

Working collaboratively with the interprofessional team to ensure an evidence-based plan of stroke care *

Provides in-person support for the clinical care of patients. Expert Stroke resource for nurses and providers. Assists with patient assessment o Coordinates patient care using defined protocols o Provides emotional support to patients and families. Provide patient and family education.

Completes NIHSS * Intervenes in real-time quality measures * Conducts chart reviews * Documents within patients' electronic health record * Collects and manages data for regulatory compliance and accreditation * Provides EMS feedback * Provides patient discharge follow-up * Collaborates with healthcare team to create discharge plan * Provides staff education * Participates in quality initiatives and research * Committee membership

MINIMUM QUALIFICATIONS:

BASIC KNOWLEDGE: Graduate of an accredited school of nursing, associate's degree in nursing required, bachelor's degree in nursing preferred Current license to practice as a Registered Nurse in the State of Rhode Island BLS certification through AHA ACLS within six months of hire NIH Stroke Scale (NIHSS) certification within six months of hire Stroke certification within 2 years of hire

Understanding of the provisions of the Nurse Practice Act of the State of Rhode Island. Demonstrated knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages, and cultural patterns in each step of the care process.

EXPERIENCE: Experienced nurse with 3-5 years of acute care experience Stroke patient care experience strongly preferred. Strong interpersonal skills to effectively function as a member of the healthcare team to coordinate and discuss patient care activities.

Pay Range:

$77,230.40-$154,460.80
