Join ChristianaCare as a Project Coordinator who will collaborate with project team members to help deliver projects efficiently. The Project Coordinator is primarily responsible for managing project-related data and administrative tasks, such as document and information distribution, report collation and communication support. The role provides opportunities to oversee small initiatives and assist in critical phases on large projects

Principal duties and responsibilities:
  • Work with various members of the project team to develop a timeline, create schedules, and oversee progress to make sure goals are met on time.

  • Prepares and presents detailed data and project related information to support organization oversight.

  • Liaise with leadership and project teams to identify and define requirements, scope and objectives.

  • Orient project leaders and team members on project management tools and processes.

  • Create and maintain comprehensive project documentation, plans and reports.

  • Monitor project progress and KPIs to ensure expected outcomes are achieved, identify opportunities for improvement and address any issues that arise.

  • Lead lessons learned and provide recommendations to improve future project delivery.

  • Assesses audience and determines appropriate level of information to be communicated in project updates.

  • Create and maintain project management calendar for fulfilling each goal and objective.

  • Documenting and following up on important actions and decisions from meetings.

  • Undertaking project tasks as required.

  • Provide administrative support as needed.

  • Social Care responsibilities - Connecting patients to community resources like physical and behavioral health services.

  • Provide culturally appropriate health education and information to patients.

Scope, purpose, and frequency of contacts:
  • Frequent contact with School Based Health Center (SBHC) leadership team, SBHC providers, SBHC SDCs,

  • SBHC OAs, MAs, Vendors and other professional staff from various system departments and community agencies.

Direction/supervision received:

Site Manager and Practice Administrator

Minimum education and experience requirements:
  • High School Graduate or equivalent required.

  • 3 years of experience in a related field. Public Health, Counseling or Social Care

  • 2 years of Community Health Worker experience required

  • Experience in Human Resources preferred.

  • Experience supporting multiple practices.

Annual Compensation Range $56,804.80 - $85,196.80 This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
