Summary
Provide intensive outreach and linkage to care case management services to person's living with HIV who are newly diagnosed, at risk of falling out of medical care, or recently released from incarceration. Services provided include assessment, service plan development, coordination of medication care referrals to internal and community supports and may take place in a clinic, hospital, and community-based clinic settings. Qualifications
Appropriate experience may be substituted for education on an equivalent basis. Minimum Required Education: Bachelor's Degree Minimum Required Experience: 1-3 years in community support setting Preferred Education: Bachelor's Degree in Public Health, Social Work, or other related field Preferred Experience: 3 or more years working with HIV patient populations; Bilingual in Spanish and English Required Certification/Licensure(s): Within first year of hire: Motivational Interviewing, Counseling Testing and Referral. Work requires occasionally lifting moderate weight materials, standing, or walking continuously. Work Environment
Occasional exposure to dust, noise, temperature changes, or contact with water or other liquids. Work is performed in an environmentally controlled environment. Sensory Acuity
Ability to detect and translate speech or other communication required. May occasionally require the ability to distinguish colors and perceive relative distances between objects. #LI-AC1
