Social Services Director

A skilled nursing facility in the northern Chicago suburbs is conducting a search for a Social Services Director.

The person in this role will oversee the social services function for the facility - including psychosocial assessments, care planning participation, discharge coordination, and family communication.

A bachelor's degree in Social Work or a related discipline is required, along with a minimum of one year of relevant experience in a healthcare or human services setting. Prior SNF director experience experience is strongly preferred.
