Transplant Coordinator (RN) - Post Lung

Caring for lung transplant recipients requires exceptional clinical judgment, coordination, and compassion. As a Transplant Coordinator RN within UVA Health's growing Lung Transplant Program, you will work closely with transplant pulmonologists, nurse practitioners, and multidisciplinary care teams to provide comprehensive outpatient care for lung transplant recipients. The Role

As the Post-Lung Transplant Coordinator, you will serve as a central point of contact for patients throughout their post-transplant journey, helping to ensure continuity of care while managing complex clinical situations in an outpatient environment. This role blends direct patient interaction, care coordination, education, triage, and collaboration with specialty providers.

In an average day, you will:

• Coordinate care for lung transplant recipients across the outpatient continuum.
• Partner directly with transplant pulmonologists, nurse practitioners, and interdisciplinary team members to support individualized patient care plans.
• Triage patient concerns and assess clinical issues related to rejection, infection, medication management, and other post-transplant complications.
• Serve as a clinical resource for patients and caregivers, providing education and guidance throughout recovery and long-term follow-up.
• Support ongoing monitoring of medically complex patients and help facilitate timely interventions when concerns arise.
• Manage patient calls, care coordination needs, and communication among multiple healthcare providers.
• Build meaningful long-term relationships with transplant recipients and their families during an important chapter of their lives.

MINIMUM REQUIREMENTS

Education: Bachelor of Science in Nursing from an accredited nursing program required.

Experience: 2 years of relevant experience.

Licensure: Licensed to Practice as a Registered Nurse in the Commonwealth of Virginia. American Heart Association (AHA) Health Care Provider BLS certification required. Relevant specialty board certification strongly preferred.

PHYSICAL DEMANDS

Job requires standing for prolonged periods, frequently traveling, bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
