This APP will practice in a busy primary care clinic, working closely with other APPs, residents, fellows, faculty attendings in the Family Medicine Primary Care Clinic. Interest in primary care including health promotion, chronic and acute illness management, the ability to work independently within a large, diverse team, an interest in intellectual/academic curiosity will be foundational to the success in this role.
Globally, the incumbent APP will be responsible for assessment, diagnosis and the development and implementation of patient-specific treatment plans. This clinician will order and interpret laboratory and diagnostic tests, as well as consult with other members of the healthcare team when appropriate. Likewise, they will guide patients and families regarding treatments. UVA's Family Medicine Primary Care Clinic at Zion Crossroads Primary and Specialty clinic is located in at Spring Creek in Zion Crossroads, VA. This is a multiple disciplinary clinic that offers laboratory, imaging and pharmacy services in addition to primary and specialty care. The full-time APP in this role will work 8 4-hour templated sessions weekly, with an allotment of 8 hours each week for non-patient facing activities (note-writing, pre-charting, follow up communication, in-basket work, etc.). The primary role is to deliver high quality evidenced based care to a diverse patient population over the age continuum. Professional support is provided by UVA Health's Center for Advanced Practice, a professional and leadership structure focused on our advanced practice clinicians. Preferred Qualifications *Experience practicing in an ambulatory/clinic setting preferred *Experience in internal medicine and primary care populations a plus *Experience in a learning environment is a plus This position is based in Zion Crossroads, VA. Clinical sessions including telemedicine will be on site. 8 hours of non-patient facing activities can be performed remotely. Background checks and pre-employment health screenings will be conducted on all new hires prior to employment. All safety-sensitive positions with direct patient interaction also require a drug screen. Administration * Partners with site Medical Director and clinic leadership to optimize patient care, operational efficiency, and access to care. * Supports implementation of enhanced care delivery models designed to improve population health outcomes and patient satisfaction. * Participates in performance improvement, data reporting, and outcomes tracking for UVA Primary Care Medical metrics. * Ensures accurate, timely documentation in the electronic medical record and adherence to UVA Health documentation and compliance standards. Professional * Demonstrates professionalism through effective communication, teamwork, and commitment to UVA Health's mission and values. * Participates in interprofessional collaboration with physicians, APPs, nurses, care coordinators, MAs, access staff, and learners to ensure seamless, team-based care. * Engages in continuous professional development and educational activities, including teaching and mentoring opportunities within the department. * Promotes health equity and inclusion in all aspects of care delivery. Clinical * Perform and document complete histories, physical exams, and pertinent findings. * Order and interpret diagnostic tests; formulate medical diagnoses based on results. * Develop, implement, and evaluate treatment plans, including prescribing appropriate interventions. * Coordinate patient flow, specialty consultations, and transfers with the care team. * Recognize and respond to emergent situations with appropriate, life-saving interventions. * Communicate and advocate for patients and families, addressing educational needs, treatment options, and age-specific considerations. * Explain and perform designated procedures after demonstrating competency while maintaining all clinical standards and patient confidentiality. Leadership * Acts as a role model to colleagues, interdisciplinary team members, the institution and to the community * Demonstrates respect to all within the institution and community * Demonstrates leadership abilities in the development, mentorship and empowerment of others within the healthcare community * Assumes leadership role in the development of the Advanced Practice Role to patients, families, other healthcare professionals and to the community * Coordinates meeting applicable leadership standards for all disciplines in collaboration with physician(s) and team members MINIMUM REQUIREMENTS Education: Graduate of an accredited graduate school of nursing or physician assistant program with a master's degree. Experience: Department and unit specific. Licensure: License or license eligible as an Advance Practice Provider (Nurse Practitioner, Physician Associate, Certified Registered Nurse Anesthetist, and Certified Nurse Midwife) as set forth by the state of Virginia. If Nurse Practitioner, license to Practice as a Registered Nurse in the Commonwealth of Virginia is required. Must demonstrate and maintain all credentialing and licensing requirements of UVA Health System, the Virginia Board of Medicine or the Virginia Board of Nursing, and appropriate certifying bodies. Current Basic Life Support (BLS) required. Current PALS/NRP, as required, specific to position. Advanced Cardiac Life Support (ACLS) as required, specific to position. PHYSICAL DEMANDS Job requires standing for prolonged periods, frequently bending/stooping, reaching (overhead, extensive, and repetitive); Repetitive motion: computer keyboard. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 50 - 100lbs. May be exposed to noise, radiation, radioactive materials, blood/body fluids and infectious disease. The starting base rate for this role is $116,123.00 annually.
