JOB RESPONSIBILITIES:

  1. Provide case management to all patients of Sixteenth Street Community Health Centers beginning at onset of prenatal care until 8 weeks postpartum. Responsibilities include but are not limited to:

    1. Perform risk assessments at the beginning of each pregnancy and as necessary.
    2. Refer patients to Sixteenth Street or community agencies/specialists as needed.
    3. Health education.
    4. Postpartum home visits as appropriate.
    5. Provide breastfeeding education and support.
    6. Supervise ancillary staff.
    7. Provide parents with newborn education and ensure appropriate scheduling of follow up care with pediatrician.
    8. Coordinate care of pregnant patients with OB providers.


  2. Comply with State of Wisconsin requirements for perinatal care coordination (PNCC) and OB Medical Home (OBMH) as outlined in the State of Wisconsin handbooks and the Sixteenth Street Policies and Procedures.
  3. Maintain appropriate patient records for the PNCC and OBMH programs. This may involve some oversight of the Prenatal Care Coordinators.
  4. Maintain the Sixteenth Street OBMH Master List and communicate effectively with the Finance Department about the patients in the OBMH program.
  5. Maintain the HMO OB Registry ensuring that it is kept up to date.
  6. Maintain BLS certification.


QUALIFICATIONS:

  1. Master's degree in Health Education or Bachelor's degree in Social Work or Nursing and 2 years of experience in health care, family services, or a community health program.
  2. Excellent oral and written communication skills.
  3. Computer and typing skills desirable.
  4. Ability to relate well to people from diverse ethnic and cultural backgrounds.
  5. Bilingual (Spanish/English) required.
