The VP, Clinical Operations is the senior day-to-day clinical leader responsible for overseeing all clinical operations, supporting field providers, and ensuring consistent delivery of high-quality, protocol-driven care. This role bridges real-time clinical decision-making with enterprise-level protocol management, quality oversight, and cross-functional coordination. This leader brings deep geriatric and home-based care expertise, an operational mindset, and the ability to support providers across a complex care environment.

Responsibilities include:

  • Clinical Operations Leadership: Serve as the operational owner for clinical workflows and care delivery processes. Design, refine, and optimize clinical workflows in partnership with operations. Integrate clinical protocols with operational systems (scheduling, logistics, documentation technology). Oversee clinical supply chain and equipment management. Act as the escalation point for urgent clinical issues and operational disruptions. Represent the clinical perspective in operational decisions and cross-functional forums.

  • Provider Supervision & Clinical Support Provide daily clinical consultation and guidance for APPs, RNs, and care team members. Facilitate complex case discussions and collaborative care planning. Serve as the primary clinical escalation pathway for acute or uncertain clinical situations. Deliver clinical mentorship, coaching, and ongoing development for providers. Remain available for urgent clinical questions via phone or text. Manage provider productivity and capacity management by monitoring performance against visit and panel-based benchmarks. Partner with operations to identify drivers of variance and implement workflow, scheduling, and coaching interventions that optimize provider efficiency while maintaining clinical quality and provider experience. Provide APP peer supervision within NP/PA licensure scope.
  • Protocol Implementation & Management Translate enterprise clinical protocols into company-specific workflows. Adapt protocols to meet RBE-specific operational and clinical requirements. Implement protocol updates and deliver provider training. Monitor protocol adherence and identify gaps requiring additional education. Maintain a structured, up-to-date protocol library and documentation repository
  • Clinical Quality Oversight Review clinical quality dashboards, metrics, and performance trends. Partner with the Clinical Quality Analyst to review chart audits and identify improvement opportunities. Deliver targeted clinical coaching to providers based on audit findings. Identify systemic clinical quality issues and lead improvement initiatives. Ensure that all clinical operations comply with relevant regulations, accreditation standards, and best practices in healthcare. Report monthly quality performance and insights to SCAN Clinical leadership
  • Care Model Implementation Partner with the Manager of Clinical Provider Operations to implement new care models. Provide clinical input on workflow design, pilot planning, and scalability. Serve as the clinical subject-matter expert on technology or workflow testing. Test and refine new protocols and deliver structured feedback to the CCO
  • RBE Clinical Support Serve as the clinical liaison for RBE-specific questions and partnership needs. Participate in RBE business reviews, joining the SCAN Clinical Leadership for major engagements. Provide clinical insights, patient stories, and outcomes data to demonstrate value. Partner with Account Managers on clinical aspects of RBE contracts and service design.
  • Cross-Functional Integration Coordinate closely with operational, clinical, product, finance, and growth partners to ensure seamless clinical delivery and alignment across the business. Partner with the VP, Operations to ensure clinical workflows, provider capacity, and operational processes are aligned and mutually reinforcing. Work with Product and Technology teams to define clinical requirements for documentation systems, workflow tools, and provider facing technology. Collaborate with Commercial Strategy & Growth to ensure clinical capabilities and needs are accurately represented in RBE engagements. Provide clinical insight to Finance regarding capacity planning, staffing models, and clinical quality economics. Partner with the SCAN Clinical team to translate clinical model priorities into product innovation and align with SCAN Group's long-term clinical strategy. Serve as the clinical voice in crossfunctional planning discussions to ensure feasibility, safety, and provider experience are consistently prioritized.
  • Team Leadership & Development Foster excellence, growth, and professional development across the provider team and broader clinical operations ecosystem. Provide mentorship, coaching, and development for APPs and clinical team members. Model a culture of clinical excellence, accountability, and collaboration. Promote retention and engagement through supportive, responsive clinical leadership. Represent the Clinical team on the executive leadership team (ELT) Partner with the CEO and other members of the executive leadership team to refine and execute the vision and strategic direction for Homebase, ensuring alignment with organizational goals
  • Support the implementation and adoption of AI-enabled tools such as intelligent call routing, conversational AI, and self-service member options.
  • Foster a positive, collaborative, and high-performance team culture.

    Qualifications and Experience:

    • Nurse Practitioner or Physician Assistant Certification

    • Strong clinical foundation

    • 7-10+ years of hands-on clinical practice in complex/geriatric care.

    • 3-5+ years of progressive clinical leadership experience with demonstrated responsibility for provider oversight, clinical workflow management, and operational decision-making.

    • Proven ability to operate at the intersection of clinical practice and business operations, with comfort translating clinical realities into operational processes, technology needs, and staffing models.

    • Demonstrated ability to lead complex clinical workflows, optimize processes, and contribute to enterprise level problem solving.

    • Experience implementing clinical protocols, driving adherence, and supporting enterprise level clinical standardization.
