Elderwood of Scallop Shell at Wakefield is seeking an experienced, passionate Director of Rehabilitation Services to lead and inspire our therapy department. This is a hands-on leadership role for a licensed Physical Therapist, Speech-Language Pathologist or Occupational Therapist who thrives in collaboration, values clinical excellence, and wants to shape high-quality rehabilitation services in a skilled nursing setting. Position Overview
The Director of Rehabilitation Services assists with ensuring the health and well-being of individuals by overseeing all aspects of the rehabilitation department. This role blends clinical leadership, operational oversight, and team development, ensuring therapy services are delivered with integrity, compliance, and compassion. You will:
Supervise rehabilitation staff to ensure high-quality therapy provision, documentation, and billing
Ensure compliance with Federal and State laws, regulations, and practice acts Assist with the hiring, onboarding, orientation, and training of therapists, assistants, and rehab aides Maintain a minimum 35% clinical productivity, staying connected to patient care Participate in required meetings, in-services, programs, and general orientation Monitor clinical, fiscal, and customer service outcomes, addressing underperformance through action planning and follow-through Foster a positive, collaborative culture that supports staff growth and resident success
Qualifications
Active Physical Therapy or Occupational Therapy license (required) Previous leadership or supervisory experience in a SNF or post-acute setting strongly preferred Strong knowledge of rehab compliance, productivity standards, and documentation requirements Excellent communication, organization, and team-building skills A leadership style that balances accountability with approachability

Responsibilities
Director of Rehabilitation Services:
Screen and provide (as necessary and as ordered by a physician) an in-depth evaluation of each individual's rehabilitation potential (unless contradicted); makes written recommendations for inter-disciplinary care planning and treatment. Inform individual or qualified representative about care and treatment plan and possible outcomes of treatment, or lack of treatment if resident refuses participation in a therapy program. Develop a treatment plan and administer treatments in accordance with the physician's orders and the rehabilitation goals; maintain required contact with physician. Oversee written evaluations, progress notes, care plans and discharge summaries of individuals receiving physical therapy according to state, federal and facility policies. Review each individual's status at least quarterly to determine need for revising rehabilitation aspects of care plan. Collaborate with other members of the inter-disciplinary health care team to integrate rehabilitation goals into the comprehensive care plan.
Qualifications
Director of Rehabilitation Services:

Licensed Occupational Therapist/Physical Therapist/Speech Language Pathologist professionally educated in an accredited curriculum with a baccalaureate degree required. -
State specific license to practice required. Management experience preferred. -
Experience in the geriatric field of Occupational/Physical Therapy or Speech Language Pathology preferable. - Current, valid CPR/BLS certification.

Experience with computer hardware and software including Microsoft Office preferred. -
Possession of ability to teach, inspire and motivate others; leadership qualities; ability to organize workload, delegate assignments and promote teamwork required.
- This position requires regular interaction with residents, coworkers, visitors, and/or supervisors. In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.
