Job Summary


The Juvenile Justice Patient Navigator serves as a non-clinical resource, advocate and liaison for patients and their families and is an integrated member of the JJC team. Under the guidance of a clinical supervisor, the Patient Navigator works in a non-judgmental and respectful manner, closely with JJC partners, to ensure patients are provided with exceptional care in the facility and upon re-entry.

The final salary and offer components are subject to additional approvals based on UC policy.

Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at UCSF. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

The salary range for this position is $29.99 - $48.23 (Hourly Rate).

Required Qualifications


  • History of working with social service experience and Medi-Cal.Commitment to the mission of care coordination, including as stipulated by CalAIM
  • Empathetic and calm approach to supporting patients and families
  • Ability to build relationships with patients, families, law enforcement partners, and providers within and outside of the Juvenile Justice Center
  • Ability to communicate concisely and courteously with patients, families, and partners
  • Ability to work closely with interpreters, as needed
  • Flexible and adaptable in response to changing patient and provider needs
  • Organizational skills (e.g., with confidential patient materials, charting, emails, and appointment tracking)
  • Interest in working with justice-involved youth and families
