Part Time Social Services Specialist l 40 hours/2 weeks l Day time hours, 8am - 4:00pm l No weekends and no holidays Ecumen Lakeshore-The Fountains provides short-term rehab and post-hospital care for up to 60 patients in spacious, private rooms along the picturesque shore of Lake Superior. The Social Services Specialist is responsible for providing social work services to residents under the supervision of the Licensed Social Worker. This position partners with the care transitions team to support residents and their family members providing general case management, and psychosocial and environmental support. The targeted pay range for this job is $26.00 - 29.00 per hour. Assists in discharge planning including referrals, communicating with outside agencies and creating a safe plan with residents and families/caregivers. 2. Assesses the resident's psychosocial needs, in the initial admission process, as related to nursing home adjustment, family involvement, behavior issues, and any other pertinent emotional needs. Incorporates assessment information into the resident's care plan. 3. Carries out evaluations, including family dynamics, caregiver abilities, and communication patterns, neglect or abuse and plan interventions based as findings warrant. 4. Participates in the development of the individualized plan of care, involves the resident and family, and attends regularly scheduled interdisciplinary group meetings, and assists the team in recognizing the effects of psychosocial stresses. Maintains clinical records on all residents admitted. 5. Documents psychosocial issues in the multidisciplinary notes in resident's charts. 6. Refers out to appropriate individual and agencies as necessary to deal with emotional behavioral issues (i.e. Chaplain, Life Enrichment, Psychologist, Ombudsman). 7. Possesses a basic knowledge of the components of the Patient Self - Determination Act and is available to facilitate the completion of these forms as the situation warrants.
