Overview
The HCR Integration Manager for Vermont's Rural Health Transformation Program (RHTP) will lead the statewide planning, coordination, and implementation of Vermont's RHTP award and oversee all program activities, including the Shared Services initiative. This role designs and manages multiple workstreams-including 35+ grants and contracts totaling over $195 million a year for the next five years to support Vermont's health care transformation goals while ensuring full compliance with CMS requirements. The Director develops project plans, timelines, budgets, and reporting structures; oversees contractors and staff; coordinates federal grant management; and ensures transparent progress monitoring, fiscal accountability, and measured outcomes aligned with the RHTP narrative. Environmental Factors
Duties are performed primarily in a standard office setting. Private means of transportation must be available for occasional travel. The incumbent must be able to reconcile a broad array of competing interests within programmatic and budgetary constraints. Significant pressure from deadlines may be anticipated. Minimum Qualifications
Five (5) years of experience of managerial experience in project development. OR Bachelor's degree in data science, informatics, public administration, policy, or human services related field AND three (3) years of experience of managerial experience in project development. OR Master's degree in data science, informatics, public administration, policy, or human services related field AND two (2) years of experience of managerial experience in project development.
