Social Worker - Case Management - 40 hours - Days Eligible for Full-Time Benefits Shift: 7:30am - 4:00pm General Summary The Case Management Social Worker coordinates the overall interdisciplinary plan of care for patient, from admission to discharge. Responsibilities: Creates plan for care across the continuum, integrating patient/family preferences and values Serves as a resource to assess, evaluate, recommend and treat legal, neglect, substance abuse, crisis, psychosocial and various adjustment needs Advocates for resources and removal of barriers Maintains ongoing dialog with supervisor and care transition team members to ensure effective implementation and reevaluation of health plan Acts as a resource for adequate medical record documentation, complex level of care recommendations, and services as they relate to diagnoses, and treatment options for post-discharge care EDUCATION/EXPERIENCE REQUIRED Social Worker credentialed from the Michigan Board of Social Work obtained prior to hire date or job transfer date required, any level is accepted High school diploma/GED with 2 years of experience, or Associate's degree, or Technical degree required Discharge planning experience Preferred Medical Terminology Willingness to learn Medical Social Work Experience with behavioral health with resources and placement is helpful Additional Information Organization: Henry Ford Providence Hospitals Department: Social Service 001 Henry Ford Health Location: Henry Ford Providence Novi Hospital Shift: Day Job Union Code: Not Applicable
