The Program Manager, Community-Based Community Engagement (CBCE) provides leadership, support, and guidance to individuals with disabilities to help them build skills and engage meaningfully in their community. This role oversees CBCE services, supervises program staff, ensures regulatory compliance, and serves as the primary liaison for consumers, families, CSBs, and community partners. The Program Manager ensures high-quality service delivery aligned with DMAS, DBHDS, CARF, DOL, HIPAA, and other applicable requirements, supporting Didlake's mission of promoting independence, inclusion, and community life engagement.

Through leadership, regulatory oversight, and strong partnerships with families, case managers, and staff, this role ensures that CBCE services are delivered safely, effectively, and in alignment with best practices.

What You Will Do



  • Conduct intake, assessments, and long-term support planning for all CBCE consumers.
  • Develop and lead Individual Service Plans (ISP) using Person-Centered Planning and in accordance with DMAS, HCBS, and DBHDS regulations.
  • Write ISPs, oversee quarterly reports, and monitor daily/weekly documentation of consumer progress.
  • Ensure full compliance with HCBS Settings Regulations, including community integration, informed choice, autonomy, and rights protections.
  • Address holistic needs across employment, medical, behavioral health, education, finances, daily living skills, housing, and recreation.
  • Educate consumers on service and employment options that promote independence and community engagement.
  • Coordinate and facilitate interdisciplinary team meetings and provide crisis intervention when needed.
  • Maintain data, records, and documentation meeting all DMAS, DBHDS, CARF, DOL, HIPAA, and Didlake standards.
  • Regularly audit consumer records to ensure accuracy and regulatory compliance.
  • Oversee daily program operations, consumer schedules, staff assignments, and service delivery.
  • Maintain communication with families, CSBs, residential providers, and healthcare professionals.
  • Provide direct consumer support as needed, including training, community life engagement, and personal care assistance.
  • Connect consumers with services, supports, assistive technology, and environmental modifications aligned with service-plan goals.
  • Track employee and consumer service time in designated systems to ensure accurate billing.
  • Conduct ongoing reviews of consumer progress through data analysis, observations, and team discussions.
  • Secure and manage service authorizations from funding agencies.
  • Supervise, train, and support program staff, ensuring effective service coordination.
  • Complete assessments, intake, referral, and discharge processes.
  • Implement clinician and physician recommendations in accordance with regulatory guidelines.
  • Oversee specialized training and support for CBCE activities (ADLs, money management, social/recreational activities, community integration, etc.).
  • Advocate for consumer rights and serve as a key resource for community referrals.
  • Assist the Director in monitoring Medicaid and Private Pay funding utilization.
  • Prepare departmental reports, program evaluation data, and outcome measurements.
  • Communicate employee or consumer concerns to leadership and collaborate with HR on recruitment, staffing, and evaluations.
  • Participate in budget development, staff meetings, corporate committees, and required trainings.
  • Follow safety protocols, including use of appropriate PPE.
