Overview
Our agency is seeking an Intake Clinical Liaison who will guide patients, families, and referral partners through the transition into home health services with clinical expertise and attentive service. This role combines clinical judgment, coordination, and relationship management to ensure each referral receives timely and accurate support from inquiry to admission.

Key Responsibilities

Referral Management
* Coordinate the transition of referrals into active admissions through clear communication with nursing, therapy, marketing, and operations teams
* Schedule evaluations, consents, and start of care visits
* Exchange essential clinical and logistical information with interdisciplinary team members
* Serve as a consistent point of contact for patients, families, physicians, and referral partners
* Maintain accurate documentation within the EMR and intake tracking tools
* Provide timely status updates on pending and admitted referrals

Compliance and Patient Safety
* Support compliance with CMS Conditions of Participation, AHCA requirements, and ACHC standards
* Protect patient confidentiality and uphold all patient rights
* Report concerns, incidents, or potential safety issues according to agency procedures
* Participate in quality initiatives directed at improving patient experience and admission timeliness

Professional Collaboration
* Represent the agency with professionalism and service-oriented communication
* Work closely with clinical, operational, and business development teams to ensure continuity from referral through admission
* Contribute to case conferences and interdisciplinary meetings
* Assist with training of new intake team members when requested

Administrative Coordination
* Communicate with pharmacies, equipment vendors, and other partners to prepare for admission
* Maintain organized records within electronic systems
* Stay informed about agency programs and service capabilities
* Manage priorities to meet regulatory and organizational timelines

Qualifications
* Active RN or LPN license in good standing
* At least two years of recent experience in home health, discharge planning, or intake coordination preferred
* Knowledge of Medicare home health eligibility and required documentation
* Strong communication skills and the ability to maintain positive relationships with referral sources
* Proficiency with EMR systems and intake tracking tools preferred
* Strong organizational and analytical ability

Work Environment
This is primarily an office-based role with regular communication and interaction across clinical teams and referral partners. The position requires the use of standard office equipment and occasional participation in after-hours coverage. Tasks do not routinely involve exposure to blood or body fluids but may require the performance of unplanned tasks where exposure could occur.
