We are seeking a Youth Family Foundations Case Manager to join CTI's Youth Services team, located at the Youth Opportunity Center. The Youth Family Foundations Case Manager will provide advanced levels of support to young adults (14-25) and their growing families entering Youth Family Foundations programming. This includes providing advanced level case management, advocacy, referral information and supporting participants in overcoming barriers towards long-term, independent housing.

Responsibilities Include:



  • Assess for general need for each family or individual
  • Connect to community supports geared towards family development
  • Increase access to nutritional supports
  • Connection to prenatal supports and in-home supportive services
  • Increasing access to education and employment
  • Creation of individualized goals with a youth-focused lens
  • Reduce barriers and overcome obstacles towards stability
  • Data tracking, entry, and submission into local and state systems
  • Support existing Youth Service programming and team members as needed


Qualifications:



  • Bachelor's Degree preferred, preferably in a Human Services related field
  • 2 years of experience working with youth and young adults preferred
  • Ability to work comfortably with pregnant and parenting youth in both group and individual settings, preferably with experience advocating for families
  • One-year assessment, advocacy and case management work in a Human Service field preferred
  • Demonstrated knowledge of community resources available to low-income individuals and families
  • Excellent customer service, organizational, oral and written communication skills
  • Ability to manage multiple priorities and work in a fast-paced environment
  • Proficient computer skills required in basic computer software and data collection systems
  • Excellent crisis management skills
  • Ability to maintain accurate records and confidentiality of information
  • Excellent attention to detail
  • Valid Driver's License
