The Senior Director leads care delivery innovation for patients. This role is a strategic business leader who fosters innovation, collaboration, and continuous improvement. This highly-visible role will work across DaVita's clinical leadership and operating teams, with accountability for formulating, testing, and implementing new programs to slow the progression of kidney failure, manage complex situations, and help patients achieve superior health outcomes.

ESSENTIAL DUTIES & RESPONSIBILITIES

ESSENTIAL FUNCTION

  • Partner across OCMO, MOC lanes, and operations leadership to build and update multi-year roadmaps that supports our models of care. 20%
  • Lead multiple MOC design focus teams (Tier 1 Interventions) to develop high-impact patient interventions. Serve as business lead that brings together multiple focus areas into one cohesive care model that applies across care teams. Provide guidance on interventions to prioritize across the model of care continuum, given staffing and operational constraints by program or market. 20%
  • Lead teams to identify high-impact focus areas. 20%
  • Refine programs, processes and interventions and prepare for full launch across multiple markets, in partnership with physician and operational partners. 20%
  • Other duties as assigned 20%
