The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.

Job Overview:

Under the direction of the VP supervision of the Regional Director of the Community Specific Program, the Director will build effective working relationships with affiliated and non-affiliated physicians, senior centers, hospital providers, and community organizations to improve individualized patient care coordination and reduce barriers across the health care continuum. Ensures compliance with all applicable policies, laws and regulations. Manage Call Center at MMC for all needed service and refer to other locations; Support other sites Chinese teams to do navigation and scheduling. Provide cross coverage and support other teams for regional community outreach. Provide needed in-service and guidance for the new FTE at other sites.

Qualifications:

Required:

  • Bachelor degree
  • Must be fluent in Chinese and English and a certified translator
  • Minimum of 5 years Manager experience
  • Must be able to handle multiple projects and delegate duties as required.
  • Must possess excellent organizational, time management and interpersonal skills.
  • Strong communication and formal presentation skills
  • Well-developed administrative abilities and leadership skills necessary.
  • Proficiency in Windows-based computer programs such as Word, Excel, PowerPoint, Outlook, and etc.
  • Strong written, telephone and verbal skills. Strong organization skills.

Preferred:

  • Strong medical terminology background
  • Experience in health related field
  • Experience in project and data managements

Essential Functions:

  • Acts as a patient advocate for Chinese patients.
  • Address physician concerns in Chinese community regarding referral process.
  • Educate physician office staff regarding hospital referral process for Chinese community. Evaluates performance of Chinese Medical Program based on following criteria; volume, revenue and patient satisfaction.
  • Identifies and addresses issues of community awareness or concern. Raises community's awareness of Chinese Medical Program.
  • Works with Public Relations and other MMC resources to improve community awareness.
  • Organize and conduct community event and health workshop.
  • Responsible for the development and operation of Community Specific Medical Program under supervision of Director, AVP and VP
  • Monitors attrition via staff feedback and exit interview information, take corrective action. Monitors changes in patient population (demographics, acuity, volume, resource needs)
  • Participates in the development and/or revision of Standards of Care and Practice consistent with documented policy/procedure.
  • Problems solves daily operational issues in a proactive manner when constantly assessing workflow patterns for efficiency and productivity.
  • Seeks assistance, advice when needed to maintain operations of projects and/or resolves system barriers.
  • Shows creativity when faced with limited resources.
  • Submits requested reports in accordance with time requirements.
  • Takes action to ensure achievement of service, financial and clinical resource targets.
  • Manage Regional Call Center at MMC: Based at Monmouth Medical Center, He/She will manage all the calls from the community and refer patients to respective Chinese teams; collect and compile referral data; follow up with the respective team that the referred patients receive the care they need; liaise with community resources and providers for program and patient needs.
  • Support Navigation and Scheduling of Chinese/Russian/Indian Teams at Other Sites
  • Provide Cross Coverage and Support for Other Chinese/Russian/Indian Teams with Regional Community Outreach
  • Provide needed in-service and guidance for new FTE at other sites
