Reasons Eating Disorder Center (Reasons) is seeking candidate professionals, preferably with previous experience working with eating disorders, trauma, substance use and LGBTQIA+ communities to provide compassionate care to both adult and adolescent patients in our Reasons Inpatient eating disorder program at BHC Alhambra Hospital.

The Case Manager Associate is responsible for assisting the Reasons Eating Disorder Center Case Management Department in various administrative duties, responding to referrals and requests for Case Management services and information and for providing technical assistance to the Case Management staff in the utilization review and discharge planning process.

Key Responsibilities Include:

  • Assist in obtaining the appropriate patient documents and records.
  • Arrange disposition and community resources, as directed by the Director of Case Management or the Case Manager, and communicate with the designated treatment coordinator.
  • Maintain reports, statistics and other data, and support the Outcome System.
  • Maintain good communication and relations with the Case Manager, Psychiatrist, therapists, registered dietitians and other staff.
  • Establish an effective relationship with customers, ensuring the appropriate customer service is provided.
  • As directed by Case Manager, coordinate and communicate discharge planning needs to patient/family/significant others and the treatment team.
  • Document discharge planning activities as required.
  • Gather information from patient/family/significant other and treatment team to help identify social work and discharge planning needs.
  • Attend and participate in treatment planning sessions as necessary and as directed by Case Manager.
  • Assist the psychiatrist in scheduling meetings with patients and administrative follow up tasks.

Requirements:

Education: Associate's Degree or Bachelor's in social work, mental health or a related field preferred; or equivalent combination of education and experience.

Experience: One (1) year experience working in the behavioral health preferred.
