Job Summary: Join our Community Care Team as a Community Health Worker (CHW) to make a tangible impact on the health of complex, chronically ill residents in Howard County. You will connect individuals with essential resources, foster chronic disease management, and provide health education to help them achieve their wellness goals while minimizing preventable hospital visits. As a key member of our multidisciplinary team with Community Health Nurses and a Licensed Clinical Social Worker, you will create and monitor care plans, conduct regular home visits, and support clients through personalized interventions. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.
