Position Summary

This position provides education to patients/families about advanced care planning. Act as patient advocate or liaison between patient, family, and healthcare personnel to ensure appropriate patient utilization of advanced care planning services. Consults with patients/families upon referral for advanced care planning. In addition, this position is responsible for the delivery of pastoral care to patients, their families, students, and staff as a member of the Pastoral Care team.

Duties & Responsibilities
  • Provide education to patients/families about advance care planning.
  • Act as patient advocate or liaison between patient, family and healthcare providers to ensure appropriate and utilization of advance care planning services.
  • Consult with patients/families upon referral for advance care planning.
  • Provide patient consults and follow-up regarding Advance Directives upon referral. Provide additional referrals or resources, as appropriate.
  • Document consultation outcomes and retain records per Pastoral Care process.
  • Plan, implement and evaluate the advance care planning program activities in order to educate community agencies, physicians, other health care personnel, and the general public as to the programs and services provided through advance care planning.
  • Responsible for community engagement strategies and activities as well as oversight of volunteers involved in these functions.
  • Responsible for advance care planning program development. This person will develop, write and implement goals and objectives to maintain the highest standards for advance care planning.
  • Provide pastoral ministry (in a timely manner) to a variety of individuals, taking into consideration multiple elements of cultural and ethnic diversity and social conditions, without imposing one's own perspectives.
  • Assess patient and family spiritual needs and assist families in using their faith in dealing with illness, trauma, and stress; visit newly admitted patients and patients with special requests; visit critical care areas; follow up with patients from previous visits as appropriate.
  • Provide crisis intervention as necessary.
  • Provide pastoral counseling to patients and families regarding religious/spiritual and emotional consequences of illness and/or disability, accessing and mobilizing family/community spiritual resources to meet identified needs. Provide crisis ministry and bereavement/grief ministry to patients, families, and clinical personnel.
  • Provide a range of skills, including empathic reflection, listening/attending, and appropriate use of religious/spiritual resources.
  • Maintain a working relationship between Pastoral Care and medical staff, nursing staff, and other departments.
  • Contribute to the department's plan for continuous quality improvement (CQI).
  • Support an interdisciplinary approach to pastoral care and advance directives consultation.
  • Participate in appropriate committees.


Qualifications:

Minimum Qualifications
  • Master's degree from an accredited theological school or religious studies, or equivalent, ordination or
    commissioning for ministry by a recognized religious group, and a current endorsement for chaplaincy by a
    recognized religious group required.
  • Minimum of one (1) year clinical chaplaincy experience required with knowledge/background in Advance Directives.
  • Additional training, education, experience in Advance Directives preferred.
  • Formal ethics training and education are preferred.
  • Eligible for certification as Board Certified Chaplain, with completed board certification preferred. (Must have 4 units of CPE. Certification must be obtained within 18 months of hire.)
  • Awareness of self as pastoral caregiver, including attitudes, values, assumptions, strengths and
    weaknesses, and the ways that ministry affects people.
  • Knowledge of the hospital/clinic system and multi-disciplinary teams.
  • Empathy for the problems experienced by patients and families; commitment to help the distressed. The
    interpersonal skills needed to work with distressed and anxious patients, families, and staff.
  • Demonstrated computer literacy; ability to document & retrieve information through computer applications.
  • Ability to work independently, and exercise good judgment.
  • Possess excellent interpersonal skills to establish and maintain professional relationships with
    members of the health care team
  • Must be able to work Tues - Friday (0930 to 1800) and Saturday (0700 to 1530) or Sunday (0700 to 1530) and Mon - Thurs (0930 to 1800)
