Lead Care Manager II (Enhanced Care Management)

This is a full-time, hybrid position that requires a mixture of field work, office work, and time spent working remotely. The primary working location will be the Inglewood and the surrounding area.

You have the option to elect to a 9/80 flex work schedule after the first six months of employment.

The pay range for this role is between $23.41 - $26.93 per hour.

$7,500 sign-on bonus for external candidates.

Summary

As a Lead Care Manager II, you will provide case management, crisis intervention, and community resource coordination for individuals with mental health and/or substance use needs. You'll guide members through integrated care services, promote recovery and dignity, and mentor new team members. You will also conduct assessments, verify eligibility for Enhanced Care Management (ECM), and oversee care planning and service delivery to ensure members receive comprehensive, person-centered support.

Primary Duties



  • Mentors new employees joining the agency especially in onboarding for new staff.
  • Attends trainings relevant to the role; presents information in team meetings and leads presentations and/or trainings to staff.
  • Serves as a content specialist that may tap into skillset and/or specialty presenting relevant information and knowledge to peers and other colleagues.
  • Assists with other division projects as needed.
  • Some elevated duties, assignments, and projects in supporting and working with CC3.
  • Assist member with maintaining enrollment eligibility for enrollment in ECM.
  • Assists members to locate, access and utilize a full range of health care services, community support groups and resources; establish and maintain housing; access mental and medical care, rehabilitation programs and drug/alcohol services; increase social contacts and engage in constructive activities.
  • Coordinate with member's treatment team to coordinate care and transitional support.
  • Assesses members' needs and functioning, presents cases in treatment team meetings and in case conferences and monitors progress as needed.
  • Acts as member advocate interfacing with housing and support agencies including property owners, support groups, Social Security, Medicare/MediCal, etc.
  • Develops and maintains rapport and cooperative relationships with community agencies for the benefit of the members.
  • Communicates and collaborates with providers and care teams, regarding the members' progress and care needs.
  • Facilitates care transitions between providers, partners, and referral sources and specialty care providers.
  • Interfaces with clinical and medical staff in coordinating member care.
  • Provides crisis intervention and facilitates conflict resolution among the program members as needed; assists members in problem-solving life situations.
  • Meets Agency member service expectations.
  • Monitors and maintains documentation of member services; ensures compliance with Agency, county, state and federal policies and procedures, standards and guidelines.
  • Develops and maintains clinical skills through seminars, workshops, or other forms of continuing education/training related to individuals with a history of mental illness and/or substance use disorders.
  • Attends and participates in Agency and program/team meetings, individual/group supervision, and related community Agency meetings as required/requested.
  • Provides social rehabilitation services to members and their families individually and/or group setting.
  • Conducts outreach services to generate member referrals.
  • Participates in community resource and/or health fairs.
  • Provides parenting services to individuals, groups or the community at large.
  • If bilingual, assists with interpretation services for medical staff.
  • Performs other related duties as requested.


Position Requirements



  • Bachelors degree preferred OR 2 years of case management experience.
  • For outpatient DMH programs, a minimum of two (2) years related experience may be substituted for a bachelor's degree.
  • For substance use programs, a degree may be substituted for experience, however, CAADAC or CAADE certification is required, with or without a degree.
  • Depending on the program, may require lived experience which could include navigation of the mental health system and/or substance use treatment system as a consumer or a certificate as a Medical Assistant.
  • Above Expectations in the last Performance Evaluation Cycle.
  • Current knowledge of all job specific skills including crisis intervention and multicultural and socio-economic issues, and documentation standards.
  • Knowledge of Agency policies and procedures, HIPAA, policies and documentation guidelines, and county, state and federal regulations relating to service programs for individuals and families with a history of mental illness and/or substance use.
  • Knowledgeable about and actively supports integrated mental health, physical health and substance use treatment for improving care for the whole person.
  • Basic advocacy skills include a holistic approach to integrated health care and case management.
  • Skills to present ideas, information, and viewpoints clearly, both verbally and in writing.
  • Able to efficiently use the personal computer to include Microsoft Office Suite and Electronic Health Record.
  • Ability to travel in the community and/or between agency sites to provide services in a variety of settings, including outpatient and field-based locations (home, school, etc.).
  • Ability to utilize data, outcomes, experience and judgment to make effective business and therapeutic decisions.
  • Demonstrate commitment to team objectives and Didi Hirsch mission.
  • Adapt to changing needs by acquiring new skills and knowledge.
  • Ability to transport members using personal or Agency vehicle and/or public transportation.
  • Current California driver's license, car insurance, and a driving record acceptable to the Agency's insurance carrier.
