The purpose of the Clinical Social Worker 3 (MSW) position is to provide both direct patient care to patients and families and assist in the coordination of team delivered services. The Clinical Social Worker 3 will complete a psychosocial assessment; develop treatment plans that are congruent with the overall health care plan; provide psychosocial intervention; educate patients, their family members, staff and social work students; facilitate compliance with treatment; work with the community to provide needed services; and coordinate discharge planning activities. The Central Virginia Center for Coagulation Disorders (CVCCD) is looking for a new clinical social work to join their team! This hemophilia treatment centers' goal is to provide multidisciplinary care for both pediatric and adult patients/families with bleeding disorders. Individualized treatment plans including support for psychosocial barriers to care is integral for these patients. This team will rely on this clinician to help mitigate barriers to treatment including educating patients/families about their diagnoses, assessment and coordination of resources including for mental health and substance use treatment needs, providing crisis intervention in clinic, coordinating with schools, engaging patients in advance care planning conversations, etc. Additionally this role includes assisting patients with understanding and navigating health insurance; Ideal candidates will have familiarity with health policy, including the 340b program, entitlement programs, and the private health insurance market. This clinical social worker will meet directly with patients and their caregivers face to face primarily in clinics on the downtown main campus with their team, with periodic day travel to satellite clinic locations. Work will also include close collaboration with the Virginia Hemophilia Foundation and Hemophilia Association of the Capital Area. This new team member will join a team of social workers in our Care Coordination Department who support various clinics throughout the Central Virginia area.
