Overview

This is a hybrid role (field and remote) requiring travel within the Polk County territory. Schedule: Full-time, Monday through Friday, 12:00 PM - 9:00 PM.
Compassionate Care Hospice, an Amedisys Company, is seeking a dedicated RN Admission Nurse to join our exceptional team in Lakeland. Attractive Compensation
Salary Range: $80,000-$85,000 annually, with additional bonus opportunities.
Final compensation may vary based on location, job-related knowledge, skills, and experience.

Orientation, Onboarding & Training

You will then continue your on-boarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.


Responsibilities

  • Coordinates admissions of hospice patients by making skilled assessments of patients initial physical, psychosocial and spiritual needs.

  • Coordinates the establishment of the initial plan of care with the attending physician, medical director and the interdisciplinary team.
  • Provides skilled interventions and instructions to patient and caregivers to provide optimum symptom control and patient comfort under the direction of physician.
  • Functions as a liaison between disciplines and facilities during the admission process.
  • Develops, prepares and maintains individualized patient care progress records with accuracy, timeliness and according to policies. Submits accurate documentation per care center policy. Records pain/symptom management changes/outcomes as appropriate.
  • Communicates significant findings, problems, changes in condition or environment to the clinical manager/physician and other members of the hospice care team. Communicates with the physician regarding the patients' needs and reports changes in the patient's condition; obtains physician orders as required.
  • Performs on-call responsibilities and on-call services to patients/families as assigned.
  • Performs other duties as assigned.

Qualifications

  • Current RN license, specific to the state(s) you are assigned to work.

  • 1+ year(s) of clinical RN experience in healthcare setting(s) - REQUIRED.
  • Current CPR certification.
