Join our collaborative and mission-driven team as a Discharge Coordinator, where you'll be the bridge between high-level hospital care and a smooth, confident transition home for our tiniest patients and their families.

About the Role:

As a NICU/Newborn Discharge Coordinator, you'll play a pivotal role in ensuring the continuity of care from hospital to home. You'll coordinate discharge plans, provide essential education and guidance to families, collaborate closely with the multidisciplinary care team, and facilitate home health referrals with efficiency and empathy.

What You'll Be Doing:
  • Coordinate all aspects of discharge planning for patients in the NICU, Newborn Nursery, and Mother-Baby units.

  • Serve as a knowledgeable resource and consultant for both families and the healthcare team.

  • Manage seamless transitions to home health agencies, ensuring families feel supported and informed.

  • Facilitate communication between care providers, families, and community resources.

  • Participate in performance improvement initiatives and help collect key data to enhance unit operations and patient outcomes.

What You Bring:
  • RN Licensure in Missouri (Required)

  • Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN)

  • Current BCLS & NRP certification

  • 3+ years of nursing experience with a strong foundation in Mother-Baby, Pediatrics, or NICU

  • Exceptional communication and interpersonal skills - you can connect with families and team members alike

  • Top-notch organizational and problem-solving skills - you think ahead and stay calm under pressure
