Peer Specialist - In Reach

POSITION SUMMARY

The In-Reach Peer Specialist connects with people in various settings and engages them with Social Practice informed groups that they run with a Fountain House staff. The In-Reach Peer Specialist bridges these people to services like Fountain House, Ryan Chelsea Clinic, Care Management and the Bronx Clubhouse.

Shift: Part-time hours up to 15 hours per week.

Salary: $22.95 per hour.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Responsible for the co-facilitation of Social Practice informed groups at settings such as MPC, Bellevue Hospital, and homeless shelters.
• Develops therapeutic relationships that can be leveraged to connect people with services.
• Responsible for traveling to and from these settings and bringing supplies that will be utilized in groups.
• Maintains records pertinent to the groups, and assists people interested in joining Fountain House with applications to the clubhouse.
• Completes tours for potential members when they visit the clubhouse. Travels with potential members to and from the clubhouse.
• Accompanies potential members to and from appointments, giving them support and helping them to get home safely.
• Provides services in a person-centered, goal-oriented, trauma informed, and culturally competent approach which includes advocacy, outreach and engagement, self-help tools, recovery supports, pre-crisis and crisis supports.
• Perform related duties as assigned by supervisor.

REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES

• Knowledge of Medicaid, Social Security, and other entitlement systems.
• Excellent interpersonal skills required.
• Knowledge of Social Practice or experience with the clubhouse model.
• Ability and willingness to regularly travel using public transportation throughout the boroughs and have flexibility in hourly/weekly schedule.
• Must be able to work under pressure and meet deadlines, while maintaining a positive attitude and providing exemplary customer service
• Ability to work independently and to conduct assignments to completion within parameters of instructions given, prescribed routines, and standard accepted practices.

REQUIRED EDUCATION, CERTIFICATION, AND CREDENTIALS

• High School Diploma or GED required; Bachelor's Degree preferred.
• Lived experience required: personal experience living with a disability, history of mental health challenge and/or substance abuse and willingness to draw upon this experience to serve as a role model to support the recovery goals of individuals served.
• NYS Peer Certification (Provisional or Full Certification) not required but preferred.
