Summary:

The RN Discharge Planner is responsible for coordinating and managing the discharge process for patients, ensuring that they are prepared for a smooth transition from the healthcare facility to their home or another care setting. The role focuses on patient education, discharge planning, and ensuring continuity of care. The Discharge Planner works closely with the interdisciplinary healthcare team, patients, and their families to create a safe and effective post-discharge plan.

Qualifications:

* Bachelor's Degree in Nursing.

* Registered Nurse in the State of Georgia.

* BLS from American Heart Association.

* Minimum of 3 to 5 years of clinical nursing experience working with Cardiology and Acute Care patients.

* Previous experience with discharge, patient education, scheduling appointments highly preferred.

* Experience with managing data and EHR highly preferred.

* Excellent written and verbal communication skills.
