Position Description:

The Medical College of Wisconsin's Department of Medicine, Division of Endocrinology & Molecular Medicine is seeking a Nurse Practitioner or Physician Assistant to provide primary or specialty medical care within defined scope of practice under the supervision of a licensed physician and in accordance with MCW/FMLH/CHW policies and procedures. This is located in Milwaukee, WI at the Hub for Collaborative Medicine, Froedtert Hospital, and Froedtert Bluemound Rehabilitation Hospital.

The Advanced Practice Provider (APP) works as part of a multidisciplinary health care team, providing a broad range of diagnostic and therapeutic services for the patients of the inpatient diabetes service in the Division of Endocrinology & Molecular Medicine.

The APP will implement and manage a plan of care in collaboration with a multidisciplinary team including faculty, APPs, nurses, pharmacists, dieticians, educators, case managers, and social services

In this role you will be responsible for the following:

  • Diagnose and manage diabetes including type 1 diabetes, type 2 diabetes, gestational diabetes, and other type of diabetes mellitus.
  • Prescribe and adjust a variety of diabetes medications including insulin, non-insulin medications and insulin pumps. Adjust glycemic management and coordinate diabetes self-management education with the Froedtert Diabetes Care Center.
  • Perform history, physical exam, and laboratory testing to diagnosis and monitor complications of diabetes. Refer to specialists for further management of complications.
  • Counsel in diet for weight loss and glycemic control. Counsel for other behavior change including smoking cessation and starting and maintaining an exercise program.
  • Attend required training and meets key regulatory requirements including CMS and the Joint Commission.
  • Evaluate quality outcomes for patients in the care of endocrine service.

Core Advanced Practice Responsibilities:

  • Performs comprehensive histories and physical exams, identifies normal and abnormal findings, including physical, cognitive and psychosocial status.
  • Orders and interprets laboratory and diagnostic tests.
  • Develops pharmaceutical and non-pharmaceutical treatment plans.
  • Monitors effectiveness of plan of care.
  • Educates patients on disease and treatment plans.
  • Performs procedures as trained and appropriately credentialed.
  • Documents within the Electronic Medical Record (EMR) and completes other necessary documents.
  • Collaborates and/or consults with physicians and other health care providers as appropriate.
  • Educates, counsels, and assists patients and families regarding current health problems, health maintenance activities and preventive behaviors.

Preferred Schedule:




Scheduling flexibility required to provide for an appropriate level of patient care coverage. Requires some evening or weekend hours based on business



Position Requirements:

Experience:

  • Zero years of experienced required
  • Preference for years and type of experience may be identified by department

Minimum Education:

Advanced Practice Nurse: Master's Degree or higher in Nursing
Physician Assistant: Bachelor's Degree or higher in Physician Assistant Studies

Minimum Requirements:

Advanced Practice Nurse

  • Wisconsin RN license
  • Wisconsin Advance Practice Nurse Prescriber License
  • Certification as Advance Practice Nurse
  • Certification as Adult Nurse Practitioner preferred
  • Will consider acute care certification or family practice certification
  • BCLS

Physician Assistant

  • Wisconsin PA license
  • Certification by NCCPA
