The Patient Safety & Quality Consultant is a thought partner with VCUHS executive, physician, nursing and ancillary leaders in strategic planning and implementation of data-driven initiatives to achieve clinical excellence at the system, clinical service and care site level. The role may serve as a coach, facilitator, or advisor as appropriate for the needs of the organization. The Safety & Quality Consultant II serves as a subject matter expert in established performance improvement techniques and strategies. The role is responsible for accelerating the successful management of change to ensure improvements are spread and a sustainability plan is implemented. The Patient Safety & Quality Consultant manages improvement projects and multi-disciplinary teams through all phases, from inception to implementation and spread. The role provides formal and just-in-time education on safety & quality improvement methodologies to increase organizational improvement capability.

Employment Qualifications

Required Education:

Bachelor's Degree in Nursing, Healthcare Administration, Business Administration, or related clinical or academic field

Preferred Education:

Master's Degree in healthcare field (e.g. MSHA, MPH, MSN, MHA), Quality Engineering, Business Administration or related field

Supplemental course work in quality improvement, such as quality methodologies/tools, data analysis, patient safety, Lean, Six Sigma, quality engineering, and/or project management

Course work in statistics

Fellowship in Patient Safety

Licensure/Certification Required:

Current license in the Commonwealth of Virginia (e.g. Registered Nurse, Pharmacist, Nurse Practitioner, Registered Dietitian or similar) if candidate holds a professional license

Licensure/Certification Preferred:

Certified Professional in Healthcare Quality (CPHQ) preferred
Certified Professional in Patient Safety (CPPS) preferred
Certified Project Manager (PMP) preferred
Lean Six Sigma Black Belt preferred

Lean Six Sigma Green Belt or equivalent within 1 year to maintain position

Minimum Qualifications

Minimum of three (3) years of work experience in the healthcare environment which may include hospital or clinic setting with specific experience in the following areas:

  • clinical processes

  • safety & quality improvement processes and tools

  • facilitating and/or leading interdisciplinary groups

  • leadership (formal or informal) in a healthcare organization

  • collecting, organizing, and analyzing data

  • developing and making presentations

Previous experience using a personal computer and software application to include the Internet, e-mail, Microsoft Office (Word, Excel, etc.), MS Project, Visio

SUMMARY

This position is responsible for leading VCUHS Post-Acute performance improvement projects (and/or multiple subprojects of a program) of various sizes and complexity. It resides at the intersection of PI, data, and project management. It helps orchestrate high quality care delivered across the entire continuum of health services. This leadership role demonstrates advanced project management skills by ensuring any project's scope is aligned with the stated Post-Acute business objectives. It involves setting and managing stakeholder expectations, managing and escalating issues and changes. This position requires developing and maintaining relationships with project stakeholders to obtain project objectives while working in a matrixed healthcare environment. These activities are performed independently with oversight and supervision of the Director. Collaborates with the Director of Continuum Integration and Post Acute Care Services in the implementation of post-acute improvement efforts Hospital System-wide (hospital, ED, clinics, community) and aligned with the organization's key initiatives.

This position plays a major role in the development and implementation of core VCUHS Post-Acute systems and resources. For a variety of large and small projects, position is responsible for creating a PI analytics framework, project deliverables, and quality assurance requirements to address Post-Acute opportunities and formulate documentation to support the development and implementation of the solution(s). In addition, the position will serve as the liaison between end users and Director to understand Post-Acute user needs and develop required reporting, data analysis, PI and QA capabilities. This is a high-visibility leadership position that offers an individual the ability to make a significant contribution to the organization by working successfully across the continuum of care.

The ideal candidate for this role will have significant experience in Post-Acute or related process improvement efforts, analysis and redesign expertise. The candidate will be well-versed in coordinated care across the continuum, and able to serve patients with the highest-level customer service skills along the way. The candidate must have the ability to work with cross-functional teams, manage and communicate project status, and direct and guide project team members. The incumbent is responsible for project scoping, requirements gathering and documentation, evaluation, project communication, risk assessment, problem resolution and training plans for large and small Post-Acute projects.

Skills: Requires working proficiency with MS Office Suite, MS Project; process improvement methodologies such as Lean, ISO, Six Sigma; must perform duties in a self-directed manner with moderate supervision; must demonstrate the ability to influence without authority; enthusiastic, takes initiative, good people skills; must possess a positive attitude toward customer satisfaction and be a results oriented individual; candidate must possess excellent verbal and written communication skills; organized and detailed oriented.

Primary Responsibilities:

  • Partners with administrative and clinical leaders across the continuum to identify Post-Acute process improvement opportunities to be analyzed for impact and overall value creation, present viable opportunities for consideration
  • Uses recognized PI frameworks to analyze current Post-Acute issues, possesses a strong ability to present findings to management and suggest action plans based on analysis
  • With Director Post-Acute and Post-Acute Network Manager, analyzes Post-Acute needs, documents requirements and translates needs into functional work plans
  • Reviews existing processes to understand Post-Acute functionality across the continuum and uses that information to inform future decisions and designs
  • Assists with estimating resource needs for Post-Acute business and clinical functions, equipment, other; prepares routine project status reports
  • Tracks and monitors the progress of Post-Acute project activities; reports status, risks, issues; recommends adjustments to ensure the successful completion of projects
  • With Director delivers requirements, performs analysis, and evaluates downstream clinical, fiscal, process, and/or organizational impacts
  • With Data Analyst support, the position analyzes trends, develops reports, facilitates communication and provides feedback on Post-Acute projects and programs' performance
  • Facilitates scope definition of multiple Post-Acute projects in collaboration with senior management in CCPAS
  • Coordinates and prioritizes activity with all involved stakeholders across the continuum to determine realistic scope and appropriate focus
  • Works with Director and departments to help define and document processes that will help streamline the adoption and use of Post-Acute services
  • Communicates with external partners to define/discuss/document VCUHS Post-Acute issues, concerns, needs
  • Maintains technical proficiency in project management software, enterprise systems and related applications
  • Leads teams for design, process mapping; ensures Post-Acute needs are all-encompassing and captured correctly, successfully interfacing with care paradigms across the continuum
  • Tracks and reports on Process Improvement activity underway for Post-Acute areas and projects
  • Facilitates team sessions involving the subject matter experts on specific Post-Acute processes
  • Documents and streamlines existing processes (as is) and help develop new (to be) processes
  • Maps process and analyzes and defines health system requirements in terms of Post-Acute capabilities and demands
  • Develops or assists in the development of work plans, time schedules, and project task items and schedules for Post-Acute programs across the continuum of health care
  • Performs as a true business Partner who shares ownership and accountability of projects and deliverables
  • Fosters a culture of continuous improvement and models behaviors for a leader as best in class
  • Works as part of a team Post-Acute professionals to deliver excellent support with outstanding customer service, satisfaction, and timeliness in improving care and outcomes
  • May perform other duties as assigned
