The Master's-level Social Worker (MSW) plays a vital role within Henry Ford Jackson's palliative care team, providing essential psychosocial support and care coordination for patients and families facing serious illness. This position requires a Master of Social Work degree and offers a dynamic practice environment, splitting time between our inpatient palliative care unit and community-based home visits. The social worker conducts comprehensive psychosocial assessments, facilitates difficult conversations about goals of care, and provides counseling support to help patients and families navigate the emotional challenges that accompany life-limiting diagnoses. Key responsibilities include connecting families with community resources, coordinating discharge planning, addressing barriers to care, and advocating for patients' wishes and values throughout their care journey. Working collaboratively with physicians, nurses, chaplains, and other team members, the MSW ensures that the social and emotional aspects of care are seamlessly integrated with medical treatment.
